Hepatocellular carcinoma (HCC) is the fifth most common cancer in men and the seventh most common in women. This cancer varies widely in incidence throughout the world, with rising incidence in Egypt. HCC is considered the second most frequent cause of cancer incidence and mortality among men in Egypt. This study aimed to estimate the serum levels of nitric oxide (NO) and glutathione reductase in order to evaluate their role as oxidative status markers in HCC development and progression. For this purpose, serum levels of these parameters were assessed in 50 HCC patients, and 30 cirrhotic patients in addition to 15 healthy subjects as a control group. In the present study, glutathione reductase activity showed a significant increase in HCC as compared to the control group (P= 0.019). On the other hand, no significant difference was observed between the cirrhotic and HCC patients (P= 0.492). Serum NO was significantly higher in patients with HCC than in cirrhotic patients (P= 0.001) or the control group (P= 0.001), with a sensitivity of (74%) and specificity of (88.89%) at a cut-off level of 614.1 μmol/l. While AFP, alpha-fetoprotein, at a cutoff level of 200 ng/ml had a sensitivity of (52%), the specificity was (100%). Indeed, nitric oxide was high in 62.5% of AFP-negative HCC patients. In conclusion, glutathione reductase has no role in HCC diagnosis. However, nitric oxide is a potential diagnostic marker for HCC. The simultaneous determination of serum nitric oxide and AFP gave significant improvement in the detection of HCC patients compared to that of AFP alone.

Gamma-irradiated β-glucan modulates signaling molecular targets of hepatocellular carcinoma in rats

β-glucans are one of the most abundant forms of polysaccharides known as biological response modifiers which influence host's biological response and stimulate immune system. Accordingly, this study was initiated to evaluate irradiated β-glucan as a modulator for cellular signaling growth factors involved in the pathogenesis of hepatocellular carcinoma in rats. Hepatocellular carcinoma was induced with 20 mg diethylnitrosamine/kg BW. Rats received daily by gastric gavage 65 mg irradiated β-glucan/kg BW. It was found that treatment of rats with diethylnitrosamine induced hepatic injury and caused significant increase in liver injury markers with a concomitant significant increase in both hepatic oxidative and inflammatory indices: alpha-fetoprotein, interferon gamma, and interleukin 6 in comparison with normal and irradiated β-glucan-treated groups. Western immunoblotting showed a significant increase in the signaling growth factors: extracellular signal-regulated kinase 1 and phosphoinositide 3-kinase proteins in a diethylnitrosamine-treated group while both preventive and therapeutic irradiated β-glucan treatments recorded significant improvement versus diethylnitrosamine group via the modulation of growth factors that encounters hepatic toxicity. The transcript levels of vascular endothelial growth factor A and inducible nitric oxide synthase genes were significantly higher in the diethylnitrosamine-treated group in comparison with controls. Preventive and therapeutic treatments with irradiated β-glucan demonstrated that the transcript level of these genes was significantly decreased which demonstrates the protective effect of β-glucan. Histological investigations revealed that diethylnitrosamine treatment affects the hepatic architecture throughout the significant severe appearance of inflammatory cell infiltration in the portal area and congestion in the portal vein in association with severe degeneration and dysplasia in hepatocytes all over hepatic parenchyma. The severity of hepatic architecture changes was significantly decreased with both β-glucan therapeutic and preventive treatments. In conclusion, irradiated β-glucan modulated signal growth factors, vascular endothelial growth factor A, extracellular signal-regulated kinase 1, and phosphatidylinositol-3-kinase, which contributed to experimental hepatocarcinogenesis.

Background: Hepatocellular carcinoma (HCC) is the most common primary malignant liver cancer and the sixth most common form of cancer worldwide. The number of its patients are growing all over the world as it affects half a million patients yearly. The main indication of HCC is the secretion of Alfa Feto Protein which may be normal in only 40 % of its patients. Amphiregulin protein has been identified as one of the 10-gene signatures in close association with the occurrence of liver metastasis in colorectal cancer patients. The expression of AREG in normal livers is undetectable; however, it is induced during acute and chronic liver injury AREG is an early response growth factor during liver regeneration. It also contributes to the transformed phenotype of human hepatocellular carcinoma cells. Objectives: The aim of the study is biochemical investigation of amphiregulin protein in Hepatitis C virus patients. Patients and Methods: This study involved 90 participants in 3 groups: Group (1): Control group composed of 30 healthy subjects. Group (2): Cirrhosis group which is composed of 30 patients with chronic liver disease (cirrhosis). Group (3): HCC group which is composed of 30 patients with hepatocellular carcinoma on top of HCV-related liver cirrhosis. All patients were subject to an assessment of AFP and AREG. Besides, HCC patients went clinically through a full estimation of liver biochemical profile, viral indications, and finally US and triphasic abdominal CT. Results: There is a statistically significantly higher age, Amphiregulin, AST, and INR in HCC > cirrhosis > control. AFP was statistically significantly higher in HCC and cirrhosis vs. control. Though, AFP was higher in HCC vs. control, and this difference was not statistically significant. There is a statistically significantly lower serum albumin in HCC > cirrhosis > control. WBCs and platelet counts were statistically significantly lower in HCC vs. cirrhosis. There is statistically significantly higher ALT and total bilirubin in HCC and cirrhosis vs. control, and a statistically significantly lower hemoglobin level in HCC and cirrhosis vs. control. Conclusion: The result exhibit There is the diagnostic performance is good for AREG and when use to gather with AFP is perfect.
