Evaluation of macrolinguistic aspects of the oral discourse in patients with Alzheimer’s disease

Abstract: Introduction:Alzheimer’s disease (AD) is a degenerative syndrome that impairs cognitive functioning, including speech and language. Discourse can be used to analyze language processing, which is organized into microlinguistic and macrolinguistic dimensions. Objectives:To identify the occurrence of changes in the macrolinguistic dimension of oral discourse in AD patients. Design: This was developed as a cross-sectional study. Setting: Outpatient clinic of the Behavioural Neurology Division of São Paulo Federal U… Show more

“…Memory impairment typical in AD contributes to many of these dysfunctions. For example, word retrieval difficulties may be the earliest signs of AD, 10 manifesting in changes in several language aspects, such as verbal naming, 11 speech content density and quantity, 12 accurate meaning communication, 4 pausation, and speech tempo. 13 , 14 Word retrieval is often tested using picture description tasks 15 where the participants are instructed to describe what they see in a picture.…”

“…We can expect a loss of the macro-linguistic dimension of the Discourse in cases of dementia, affecting both aspects of the macrostructure and the microstructure discoursive. (11,12) In the reassessment, we observed a considerable improvement in the patient's performance regarding the number of words and units of information (table 1). The improvement of the microstructure of the speech (number of words) was accompanied by the improvement of one aspect of the macrostructure, the production of a greater number of main information units.…”

“…Since there are no criteria for the classification of normality for the elements of discourse analysis, with consensus in the literature, we used the Oral Discourse Evaluation Index in Patients with Alzheimer's Disease (AD) proposed by Lira (11,12) . The index was developed to more effectively assess language through discourse in AD, using the least possible variables, which can better identify the change in performance.…”

“…Concerns over language impairments in AD are not new. Many studies have sought to identify impairments at different levels of linguistic processing, like phonetic 3 , phonological 4 , lexical-semantic 5 and syntactic 6 , along with deficits in discourse 7 . These studies have shown that language abilities become differentially impaired as the disease progresses.…”
