“…Rather, ICG appeared to describe the performance of the liver independent of the actual presence of the cirrhotic complications and the subsequent therapeutic interventions as proven in the second cohort undergoing immediate TIPS implantation. Thus, the modified MELD-ICG will not improve on the MELD in patients with advanced liver failure (MELD >35) and high short-term mortality, as recently demonstrated in another cohort 25. Its strength appears to lie in predicting survival in intermediate to advanced disease, similar to the MELD-Na (table 4), but it is still accurate when parameters like creatinine and sodium might be adjusted through treatment such as TIPS.…”

“…It is important to assess both total and segmental liver function before liver surgery in order to predict the remnant liver functional reserve. In clinical practice, liver function tests include the indocyanine green (ICG) clearance rate at 15 minutes [1], computed tomography volumetry [2], and clinical scoring systems such as the ChildPugh or Model for End-Stage Liver Disease (MELD) scores [3][4][5]. However, these methods offer only global assessment of liver function, which cannot assess the heterogeneous functional distribution in the liver.…”

“…However, it is virtually impossible to obtain a liver biopsy from the patients with ACLF who are with coagulopathy and have high risk for bleeding. Furthermore, ACHBLF patients with liver transplantation were usually excluded in survival analysis [37]. On the other hand, the peripheral Th17 cells profile partly reflected the dynamic changes of Th17 in liver.…”
