Increased Frequency of Circulating Th17 Cells in Acute-on-Chronic Hepatitis B Liver Failure

Abstract: These results suggested that Th17 cells as well as IL-17 might be related with disease severity and prognosis in ACHBLF patients.

“…In addition to T follicular help (TFH) cells, interleukin-17-producing CD4+ T cells (Th17) also secrete IL-21 [24,25]. The highly increased frequency of Th17 cells in PBMCs has been observed in CHB patients with severe liver damage [25,26]. So the increased IL-21-secreting CD4 T cells might not be mainly derived from TFH cells, but also from Th17 cells in hepatitis B-related acute-on-chronic liver failure and severe chronic hepatitis B as Hu's report demonstrating IL-21 mainly derived from IL-17A+ IL-21+ CD4+ T cells [22].…”

“…At the same time, Th17/IL-17 axis expresses itself, a response to a sustained proinflammatory stimulus ( 51 , 52 ). So, we can understand the broad scope of the elements of this axis on the CVH, predicting outcomes in different points of the disease, from spontaneous healing ( 53 ), response to treatment ( 23 , 54 , 55 ), occurrence of acute decompensations ( 56 ), progression to cirrhosis ( 9 , 28 ), and HCC ( 53 ), including post-transplant recurrence ( 57 ), as detailed in this section.…”

“…Beyond its effect in predicting disease severity, the Th17/IL-17 axis elements are also predictors of mortality ( 56 ). In a study including 60 HBV-infected patients (30 with CHB and 30 with ACLF), the disproportionate increase of Th17 (compared to Treg) was associated with low survival among those with ACLF ( 64 ).…”

“…Th17 levels [141, 142] and the Treg/TH17 ratio appear to have a crucial role in the occurrence, development, and outcome of HBV [142, 143] and could be used as indicators of inflammation that may predict progression to fibrosis [144]. Hence, Th17 cells can contribute to immune activation and disease aggravation in patients with chronic HBV infection [138, 145], because of the correlation of Th17 cells with serum alanine aminotransferase levels [139].…”
