Previous studies have reported inconsistent results regarding the associations between metabolic syndrome (MetS) and obesity-related indices and bone mineral density (BMD). However, no previous studies have reported these associations among hemodialysis (HD) patients. The aims of this study were to investigate associations between MetS and its components and BMD T-score in HD patients and also between obesity-related indices and BMD T-score in HD patients with and without MetS. MetS was defined according to the Adult Treatment Panel III for Asians, and BMD T-score was calculated using dual-energy X-ray absorptiometry. Eight obesity-related indices were evaluated, including lipid accumulation product (LAP), visceral adiposity index (VAI), body adiposity index, conicity index (CI), body roundness index (BRI), abdominal volume index (AVI), waist-to-height ratio (WHtR), waist–hip ratio, and body mass index (BMI). One hundred and sixty-four patients undergoing HD were enrolled, and the prevalence of MetS was 61.6%. MetS was significantly associated with high lumbar spine and total hip T-scores. Regarding the MetS components, abdominal obesity and low HDL-C were significantly associated with high lumbar spine, femoral neck, and total hip T-scores; hypertriglyceridemia was significantly associated with high lumbar spine and total hip T-scores; hyperglycemia was significantly associated with a high lumbar spine T-score, whereas high blood pressure was not associated with T-score at any site. In the patients with MetS, BMI, WHtR, AVI, and BRI were significantly associated with T-score at all sites, and high CI, VAI, and LAP were also related to a high lumbar T-score. However, these indices were not associated with T-score at any site in patients without MetS. This study demonstrated positive associations between MetS and its five components and BMD T-score among HD patients. MetS, abdominal obesity, hypertriglyceridemia, and low HDL-cholesterol were associated with low risk of osteoporosis among the HD patients. Furthermore, we found that some obesity-related indices were associated with BMD T-score among HD patients with MetS but not in those without MetS. Our study highlights the importance of BMI, WHtR, AVI, and BRI in predicting the risk of osteoporosis among HD patients with MetS. In clinical practice, they can be easily calculated through simple anthropometric measurements and routine laboratory examinations and be used to quickly and conveniently assess the risk of osteoporosis among HD patients.

Objectives Obesity and metabolic status are both modifiable risk factors of lower urinary tract symptoms secondary to benign prostatic hyperplasia (LUTS/BPH). However, the association between metabolically healthy obesity (MHO) and LUTS/BPH is largely unexplored. This study aimed to investigate the risk of LUTS/BPH among different metabolic syndrome‐body mass index (MetS‐BMI) phenotypes in a cohort of Chinese males. Methods A total of 3321 males from the China Health and Retirement Longitudinal Study (CHARLS) without history of LUTS/BPH at baseline were included into the analyses. Participants were categorized into six mutually exclusive groups according to presence or absence of MetS combined with BMI status: metabolically healthy normal weight/overweight/obesity (MHN/MHOW/MHO) and metabolically unhealthy normal weight/overweight/obesity (MUN/MUOW/MUO). Adjusted odds ratios (OR) and 95% CI of LUTS/BPH across MetS‐BMI categories were estimated with multivariable logistic regression models. Results A total of 394 (11.86%) participants developed LUTS/BPH during the follow‐up. After adjusting for age, educational level, smoking status, drinking status, and BMI change, the multivariable‐adjusted OR (95% CI) for incident LUTS/BPH comparing MUO, MHO, MUOW, MHOW, and MUN with MHN were 1.99 (1.23‐3.22), 2.04 (1.14‐3.66), 1.61 (1.11‐2.34), 1.45 (1.02‐2.05), and 0.91 (0.54‐1.56), respectively. Conclusions MHO and MHOW were risk populations of LUTS/BPH, suggesting that overweight and obesity can independently contribute to LUTS/BPH, even among metabolically healthy individuals. These findings emphasize metabolically healthy individuals may still benefit from maintaining normal body weight to prevent LUTS/BPH. Our findings also support that those recommendations for LUTS/BPH should highlight the importance of maintaining metabolic health across all BMI groups among Chinese males.

Öz Purpose:The aim of this study was to evaluate the traditional and novel obesity-related indices in the determination of metabolic syndrome in adults and to determine which marker is the better predictor. Materials and Methods: A total of 419 adults between the ages of 18-65 were included in this study. Body weight, height, waist, hip and waist circumference, and blood pressure were measured; fasting blood glucose, total cholesterol, triglyceride, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol values were analyzed. Metabolic syndrome (MetS) was defined using the International Diabetes Federation criteria. The values of 23 obesity-related indices were calculated. Results: The prevalence of metabolic syndrome is 58.7% (male 41.2%; female 67.7%). The triglyceride glucose (TyG) index has the largest area under the curve (AUC) in both men (AUC = 0.894, cutoff = 3.9) and women (AUC = 0.901, cutoff = 3.9). In men, lipid accumulation product (LAP) had the second highest determination for MetS (AUC = 0.880, cut-off = 51.1), followed by TyG-waist/hip (AUC = 0.876, cut-off = 3.7). Cardiometabolic index (CMI) (AUC = 0.872, cut-off value = 1.3) and visceral adiposity index VAI (AUC = 0.868, cut-off value = 4.1) had the second and third largest AUCs, respectively, in women. Conclusion: TyG index is the best predictor of MetS. Waist circumference could be an alternative index in large epidemiology survey due to its convenient and costefective characteristics.Amaç: Yetişkin bireylerde metabolik sendromun belirlenmesinde obeziteyle ilişkili geleneksel ve yeni indekslerin değerlendirilmesi ve hangi belirtecin daha iyi öngördürücü olduğunu belirleme amacı ile planlanmıştır. Gereç ve Yöntem: Bu çalışmaya 18-65 yaş arası toplam 419 yetişkin birey dahil edildi. Vücut ağırlığı, boy uzunluğu, bel ve kalça çevresi ile kan basıncı ölçüldü; açlık kan şekeri, total kolesterol, trigliserit, düşük dansiteli lipoprotein kolesterol ve yüksek dansiteli lipoprotein kolesterol değerleri analiz edildi. Metabolik sendrom (MetS) Uluslararası Diabet Federasyonu kriterleri kullanılarak tanımlanmıştır. Obeziteyle ilişkili 23 indeksin değeri hesaplandı. Bulgular: Metabolik sendrom prevalansı % 58,7 (erkek % 41,2; kadın % 67,7)'dir. Trigliserit-glikoz (TyG) indeksi hem erkeklerde (eğri altında kalan alan (AUC)= 0,894, kesme değeri = 9,3) hemde kadınlar da (AUC = 0,901, kesme değeri = 8,3) en büyük AUC'ye sahiptir. Erkeklerde lipit birikim ürünü (LAP), MetS için ikinci en yüksek belirlemeye sahip iken (AUC = 0,880, kesme değeri = 51,1), ardından TyG-bel/kalça (AUC = 0,876, kesme değeri = 3,7) gelmektedir. Kadınlarda kardiyometabolik indeks (CMI) (AUC = 0,872, kesme değeri = 1,3) ve viseral adipozite indeksi (VAI) (AUC = 0,868, kesme değeri = 4,1) sırasıyla ikinci ve üçüncü en büyük AUC'lere sahiptir. Sonuç: TyG indeksi MetS belirlemede en iyi öngördürüdür. Bel çevresi kullanışlılığı ve uygun maliyetiyle büyük ölçekli epidemiyolojik çalışmalarda alternatif bir indeks olabilir.
