Evaluation of donor heart for transplantation

Abstract: Cardiac transplantation is considered the gold-standard treatment option for patients suffering from end-stage heart failure refractory to maximum medical therapy. A major determinant of graft function and recipient survival is a comprehensive evaluation of the donor allograft. Challenges arise when designing and implementing an evidence-based donor evaluation protocol due to the number of influential donor-specific characteristics and the complex interactions that occur between them. Here, we present our syst… Show more

“…Last, substance abuse is often associated with HCV-viremic donors, and its effects on heart transplant outcomes are unknown, as those donors were historically avoided, particularly when they used stimulants and other noradrenergic agents. 70 Some data suggest that opioid or cocaine used by the donor does not affect post-heart transplant outcomes, although information is limited. 71 All in all, additional studies with larger samples and longer follow up are awaited to better determine the safety of heart transplant from HCV NAT+ to uninfected recipients, particularly in the long-term.…”

A disfunção precoce do enxerto, a rejeição do enxerto, quadros infecciosos, lesões renais agudas e crônicas exemplificam essas complicações.…”
