The purpose of this review was to examine and summarize data for inosine pranobex (IP) immunotherapy in cervical HPV-positive patients. Persistent or recurring cervical human papillomavirus (HPV) infection is a major cause of cervical cancer. Self-clearance and blocking of cervical HPV infection depend on the status of the host immune system. Immunotherapy helps accelerate elimination of the infection. Host immunity is involved in the development of HPV infection. Several mechanisms of interaction between the virus and the immune system have been revealed; however, the mechanisms have not been completely elucidated. A properly functioning immune system impedes HPV progress and helps clear the pathogen from the body. IP has antiviral efficacy because it modulates both cellular and humoral immunities. IP has been on the market since 1971. Nevertheless, it has seldom been administered to treat cervical HPV infections. In this review, Google Scholar, PubMed/MEDLINE, Scopus, Cochrane Library, and Research Gate were searched for the period 1971–2021. Prospective controlled trials, observational and retrospective studies, and meta-analysis and reviews on immunotherapy against HPV cervical infection were explored. Prior studies showed strong clinical efficacy of combined and standalone IP therapy in reversing HPV-induced changes in the cervix, preventing disease progression, and clearing the pathogen. IP treatment enhanced host antiviral activity against HPV, delayed or stopped cervical oncogenesis, and rapidly removed HPV from the body.

Диспластические изменения многослойного плоско-го эпителия шейки матки рассматриваются в качестве предраковых состояний, которые, как известно, могут возникать как на неизмененной шейке матки, так и на фоне патологических процессов. Доказано, что самой распространенной патологией, ведущей к развитию пред-раковых заболеваний, является папилломавирусная ин-фекция (ПВИ), передающаяся половым путем [1][2][3][4][5][6][7][8].Невиданное доселе обновление технологического обеспечения, произошедшее на рубеже XX-XXI веков, совершенствование методов диагностики вирусных забо-леваний способствовали не только выявлению истинной частоты инфицированности вирусом папилломы челове-ка (ВПЧ), но и изучению связи между инфицированно-стью ВПЧ и диспластическими процессами шейки матки [8][9][10][11][12]. Доказано, что репликация ВПЧ на начальном эта-пе приводит к возникновению воспалительной реакции в шейке матки, а в последующем -к возникновению в тол-ще эпителиального слоя атипичных клеток с различной степенью нарушения их дифференцировки, к изменени-ям слоистости эпителиальных клеток (без участия в пато-логическом процессе стромальных структур). Развивается ВПЧ-ассоциированная дисплазия шейки матки, или, пользуясь современной терминологией, цервикальная интраэпителиальная неоплазия (ЦИН), известная также как плоскоклеточное интраэпителиальное поражение e-mail: cf_university@mail.ru Цель исследования -разработка комплексного метода ведения больных с дисплазией шейки матки на фоне папилло-мавирусной инфекции, включающего деструкцию пораженной ткани в сочетании с применением медикаментозной про-тивовирусной иммуномодулирующей терапии. Материал и методы. Приведены данные анализа особенностей течения и ведения диспластических процессов шейки матки у 60 женщин Симферополя и Симферопольского района, в возрасте от 19 до 47 лет. У всех больных диспластические заболевания шейки матки сочетались с папилломавирусной инфекцией. Использованы стандартные методы диагностики. Результаты. В 67% случаев диспластические процессы шейки матки были ассоциированы с инфекцией 16-го и 18-го генотипов вируса папилломы человека. Различные степени дисплазии шейки матки (ASCUS, LSIL и HSIL) у больных с папилломавирусной инфекцией были выявлены в 50, 30 и 20% случаев соответ-ственно. Проведено комплексное лечение больных с диспластическими процессами шейки матки на фоне папилломави-русной инфекции. Метод заключался в поэтапном применении специфической противовоспалительной и иммуномоду-лирующей терапии, деструкции пораженной ткани в зависимости от степени поражения и повторной медикаментозной противовирусной иммуномодулирующей терапии на завершающем этапе. Заключение. Доказана эффективность поэтап-ного применения комплексной терапии больных с диспластическими заболеваниями шейки матки, ассоциированными с папилломавирусной инфекцией.Авторы информируют об отсутствии конфликта интересов. A healthy reproductive system is inconceivable without normal vaginal microbiota, and full-fledged treatment cannot be carried out without detailed understanding of the arrangement and functions of the human microbiome. Today superbugs are a reality, and the role of such concepts as “microbiome” and “biofilms” is already undeniable in medical practice. Every doctor understands that it is necessary to choose antibacterial drugs based on practicability, global experience and evidence-based medicine. All this clearly demonstrates that there is a need to create an authoritative source of knowledge — a handbook for practitioners. Each chapter contains up-to-date information on the impact of female microbiota on the course and outcomes of pregnancy, on the etiology, pathogenesis and diagnostics of vaginal microbiocenosis disorders, and detailed treatment regimens.
