Medication prescribing errors among hospitalized pediatric patients at Nekemte Referral Hospital, western Ethiopia: cross-sectional study

Objective Incidence and clinical outcomes of medication prescribing errors are common and potentially more harmful in the pediatric population than in the adult population. Hence, this study was aimed to assess the prevalence and types of medication prescribing errors in the pediatric wards of Nekemte Referral Hospital (NRH). Results Of 384 pediatric patients included in the study, 241 (63%) were males and 116 (30.21%) of them were aged between 1–3 years. About 241 (62.76%) of the patients were treated based on empirical diagnosis and only 10 (2.60%) pediatrics had co-morbid disease. The most category of medication prescribing error was dosing error 251 (48.6%) followed by incorrect drug selection 98 (19.0%). Being critically ill (AOR = 5.31, 95% CI = 1.80–12.31, p = 0.003), route of administration via IV (AOR = 3.98, 95% CI = 1.85–11.15, p = 0.011) and via IV + IM route (AOR = 2.22, 95% CI = 1.05–9.25, p = 0.045) as well as 4–6 medications per patient (AOR = 3.10, 95% CI = 3.43–12.42, p = 0.012) and > 6 medications per patient (AOR = 7.23, 95% CI = 3.91–21.45, p < 0.001) were independent predictors of medication prescribing errors. Antibiotics were the most common classes of drugs responsible for prescribing errors.

Introduction: A prescription is a written order to a pharmacist by a doctor, which should contain all its components so that a patient can get precise and fruitful outcomes of disease treatment. Preclinical undergraduate medical students are taught and trained to develop good prescription skills. Thus, we aimed to appraise prescription writing skills among intern doctors of Birat Medical College Teaching Hospital, Nepal. Methods: This observational cross-sectional study enrolled 63 interns of Birat Medical College Teaching Hospital. Nineteen components of a prescription according to the World Health Organization were included in this study. They were name, address, degree, registration number, telephone number, sign of the prescriber, date, diagnosis, dosage form, correct dose, the correct route, correct quantity, and correct instruction of the drug, name, address, sex, and age of the patient. Then four groups; Group A (Poor), Group B (Fair), Group C (Good), and Group D (Excellent) were divided according to the number of components included in the prescription. Results: According to the study scoring system, the maximum prescriptions were from group C (65.08%) followed by group B (34.92%). None of them were group A and group D. Components such as prescriber’s name, drug’s name, dose, route, quantity, and instruction were found in all the prescriptions whereas, components such as prescriber’s address, degree, and contact number were not found in any of the prescriptions. Conclusions: The prescription writing skills of interns of Birat Medical College Teaching Hospital is not up to the mark. There are lots of missing components in their prescriptions.

Pharmacology is most rapidly expanding science in medical discipline which leads to the development of many important drugs to treat many medical conditions that was previously untreatable. This paper focuses on the necessity of integration of pharmacology in the medical curriculum in all clinical phases and ethical aspects of medicine in terms of irrational prescribing. Irrational prescribing of drugs is a major global health problem in medical practice. Currently Pharmacology is taught at phase-II, a 2 year period in a 5 years undergraduate medical Programme in Bangladesh under the name ‘Pharmacology and Therapeutics’. Indiscriminate use of clinically inappropriate and ineffective medicines are a serious problem. Medicines in Bangladesh are inappropriately prescribed, again all kinds of drugs including controlled products are dispensed and sold out frequently without prescription. Henceforth there are regular violation of medical ethics and professionalism. As only pharmacology and therapeutics deals with the correct selection of drugs, the subject should be continued to teach up to phase-III. In addition, therapeutic discussion should be incorporated with proper logbook during internship in order to make safe and efficient prescribers. Supreme priority should be given to legal and ethical aspects of medicine. Medical professionals must uphold social order by ensuring ethical practices of medicine and appropriate role modelling especially by the faculty
