Recurrent aphthous stomatitis is the most common oral mucous ulcerative lesion with challenging treatment. Herbal medicine therapy can propose clinical efficacy and safety due to its large biological activities. The objective was to review the clinical efficacy and safety of herbal medicine therapy in terms of ulcer size, pain score, healing duration, and adverse effects in recurrent aphthous stomatitis. A systematic was conducted based on the PRISMA statement. The search was performed using four electronic databases, namely PubMed, Cochrane, Science Direct, and Google Scholar for articles published from 2016 until 2021 using specific keywords. The search was limited to randomized controlled trials (RCTs), in English, full text, and study in humans. The main outcome is expected to be ulcer size, pain score, healing duration, and adverse effects. Quality assessment of selected articles was conducted using the Quality Appraisal of Randomized Trials Checklist (Cochrane Risk of Bias tool). The methodology quality of studies was evaluated using the Cochrane Handbook for Systematic Review of Interventions and Rev Man software. Five articles were eligible for analysis. The population of the sample study ranged from 34-70 patients of 15-65 y old. The herbs used were Aloe vera (Aloe barbadensis Miller), curcumin (Curcuma longa), licorice (Glycyrrhiza glabra), tobacco (Nicotiana tabacum L.), and pomegranate (Punica granatum Linn.). This review showed that there is some evidence of the clinical efficacy and safety of herbal medicine therapy in improved outcomes of recurrent aphthous stomatitis treatment with minimum adverse effects.

Background: Recurrent aphthous stomatitis (RAS) is the most common disorder affecting oral mucosa. The present study was designed to assess the clinical effectiveness of utilizing topical hyaluronic acid (HA) against chlorhexidine (CHX) mouthwashes as a control in RAS treatment.Materials and methods: Thirty-four patients with minor RAS were included in the trial. They were equally allocated into two groups with random distribution to rinse with CHX mouthwash as a control (Group I, CHX Group) and HA mouthwash in (Group II, HA group) as a study group. Evaluation of pain intensity and ulcer size were done in all study patients at baseline, 3 days and 7 days observational times. Duration of healing period was assessed in both study groups. Results:The results demonstrated a significant lowering in pain score and ulcer size in each group. Lower mean values of these outcomes were recorded in HA group compared to CHX group at 3 days and 7 days with significant difference regarding pain score. Concerning the duration of healing a significant decrease was recorded in HA group compared to CHX group. Conclusion:In conclusion, topical HA is a safe and effective treatment option for RAS with better pain control and healing duration.

Introduction: Recurrent aphthous stomatitis (RAS) is a widespread oral mucosal disease typified by the presence of a yellowish-grey base with high margins and flanked by an erythematous halo. RAS typically crops up in the lining or nonkeratinized mucosa. It has been found that smoking and salivary cotinine (a metabolic product of nicotine) levels have a protective effect against the occurrence of RAS by increasing keratinization of the oral mucosa.Objectives: To identify the effect of smoking and nicotine level on the occurrence of RAS.Patients and Methods: the study was conducted on 92 male students studying at different Universities of Baghdad, Iraq, in the period between January to December 2018. The students were thoroughly questioned about the yearly recurrence rate and healing period of RAS. Data about smoking history; method, average daily dose, and period of the behaviour were recorded for each student. Salivary Cotinine levels were measured by the Human Cotinine ELISA Kit and utilising the competitive ELISA technique based on the protocol of the manufacturer salimetrics assay #1-2002. Results:The study subjects comprised of 92 male students. Their ages were from 18 to 25 years; the average age was 21.04 ± 2.2. Forty-two of them were light smokers, and 50 were heavy smokers. They were subdivided into subgroups according to the duration of smoking (less or more than five years). The annual recurrence rates and healing period were lower in those who smoked for more than five years than those who smoked for less than five years with a significant difference in light smokers. Salivary cotinine levels were significantly higher in a heavy smoker than in light smokers. Period of healing and yearly recurrence rates were significantly lower in those who had salivary cotinine level more than 200ng/ml than in those with a level. Conclusion:The protecting impact of smoking on RAS was solely identified when individuals were heavy smokers or smoked for longer durations of time and correlated with cotinine levels, according to the available literature.
