Evaluating the population with intellectual disability unable to comply with routine dental treatment using the International Classification of Functioning, Disability and Health

Abstract: The use of GA increases with the severity of ID. Acute and painful conditions result in more frequent use of GA. Unfavourable environmental factors present barrier to routine dental management.

“…Thus maintenance of oral health seems more related to the cognitive ability to comprehend daily oral care and cope with examination and treatment than to specific medical diagnoses. This corresponds with previous studies of the ICF profile of disabled patients with difficulty tolerating dental treatment or patients requiring treatment under general anaesthesia [16], [31]. Oral function was not identified in the top ten impairments in the current study despite the fact that a high level of oral impairment was reported with 40% of children reporting impaired chewing and 28% impaired swallowing.…”

“…The ICF has only twice been used empirically in the domain of oral health [16], [31]. Practical use of the ICF is notoriously difficult as the classification is unwieldy and deliberately exhaustive, listing over 1400 items.…”

“…In line with the findings of a previous study by Norderyd et al, children with limitations in interpersonal relations and interactions were more likely to have experience of DGA than those without such limitations 11 . The children in the international DGA group also had significantly more problems in their intellectual function, as confirmed in other studies 11,22,23 . It has been suggested that problems in intellectual function therefore could be an indicator for experience of DGA, and limitations in interpersonal relationships represents a mechanism 11 .…”

“…Another way of using the ICF is to introduce the tripartite ICF model as the theoretical background of a study, but to use only the participation concept in the empirical part (Petrovic, Markovic & Peric, 2011;Faulks et al, 2013). For example, Faulks et al (2013) give an explicit theoretical reference to the ICF distinction between activity and participation, but then shift to describing a wide range of participation aspects, from acquiring skills (Chapter 1) to community, social and civic life (Chapter 9).…”

“…An interpretation that the discussed studies actually use the participation aspect and not the ICF concept of activities is supported by the fact that they do not use the ICF concept of capacity. Instead, the term ability is used (Maeda et al, 2005;Petrovic et al, 2011;Faulks et al, 2013). In the ICF, ability is only mentioned as an unspecified term when the term capacity is introduced in order to explain the ability to perform an activity in a socalled standard environment.…”
