Evaluating the efficacy of anti-VEGF therapy in patients with exudative age-related macular degeneration and concomitant glaucoma

Цель исследования-оценить влияние интравитреального введения анти-VEGF-препаратов «Ранибизумаб» и «Афлиберцепт» на функциональное состояние зрительного пути при неоваскулярной возрастной макулодистрофии (нВМД) в сочетании с первичной открытоугольной глаукомой (ПОУГ) с помощью метода регистрации зрительных вызванных корковых потенциалов (ЗВКП). Материал и методы. Обследовано 54 пациента (54 глаза) с нВМД и ПОУГ (основная группа), среди которых 24 пациента (24 глаза) с ПОУГ IA стадии, 23 пациента (23 глаза) с ПОУГ IIA стадии, 7 пациентов (7 глаз) с ПОУГ IIIA стадии. Группа контроля состояла из 39 здоровых пациентов (39 глаз). Всем пациентам основной группы ежемесячно в течение 3 мес выполняли интравитреальное введение анти-VEGF-препаратов: 35 пациентам-ранибизумаб, 19 пациентам-афлиберцепт. Офтальмологическое обследование включало специальные методы исследования: оптическую когерентную томографию (ОКТ) сетчатки с помощью томографа SPECTRALIS («Heidelberg Engineering GmbH», Германия) и ЗВКП на реверсирующий шахматный паттерн (ПЗВКП) EP-1000 Multifocal («Tomey», Германия). Все исследования проводили до введения анти-VEGFпрепарата и после 3-й инъекции. Результаты. После 3-й интравитреальной инъекции анти-VEGF-препаратов максимальная корригированная острота зрения (МКОЗ) увеличилась до 0,46±0,1 (p=0,001). По данным ОКТ, толщина центральной сетчатки в среднем уменьшилась на 110,6 мкм, общий объем сетчатки-на 1,3 мм 3 , общая толщина СНВС-на 3,8 мкм (p<0,05). По данным ПЗВКП, отмечены уменьшение пиковой латентности и увеличение амплитуды компонента Р100 ПЗВКП. Статистически значимого различия в динамике показателей ПЗВКП при антиангиогенной терапии препаратами «Ранибизумаб» и «Аафлиберцепт» не обнаружено (р>0,05). Заключение. Интравитреальное введение анти-VEGF-препаратов «Ранибизумаб» и «Афлиберцепт» не оказывает отрицательного влияния на функциональное состояние зрительного пути пациентов с неоваскулярной формой возрастной макулярной дегенерации и первичной открытоугольной глаукомой. Интравитрельные инъекции анти-VEGF-препаратов «Ранибизумаб» и «Афлиберцепт» можно считать безопасным методом лечения пациентов с неоваскулярной формой возрастной макулярной дегенерации в сочетании с первичной открытоугольной глаукомой.

PURPOSE. To evaluate the effect of elevated intraocular pressure (IOP) after multiple intravitreal injections (IVI) of brolucizumab on changes in the retinal nerve fiber layer (RNFL) and perfusion of the optic nerve head.METHODS. The study included 20 patients with newly diagnosed exudative form of age-related macular degeneration (AMD). All patients underwent IVI of brolucizumab. IOP measurements were taken with an ICare Pro tonometer before IVI, one minute after IVI, then after 30 minutes and 180 minutes. Thickness of the peripapillary RNFL was measured using Spectralis OCT (Heidelberg Engineering, Germany). The optic disc was examined using OCT angiography on Revo NX (Optopol Technology SA, Poland). All studies were carried out before the start of treatment, after one month, after the third injection, and one year after the start of treatment.RESULTS. Analysis of peripapillary scans in a patient with a history of multiple IVI a year after the start of treatment with brolucizumab showed a statistically significant decrease in perfusion density and fractal dimensions (skeleton) in the entire optic nerve head (ONH) (p<0.001), in the inner ONH area (p<0.001, p=0.01, respectively), in the nasal sector (p=0.03, p=0.04, respectively), temporal sector (p<0.001) and inferotemporal sector (p=0.03) according to OCT angiography. It was determined that a greater increase of IOP one min after IVI was associated with a greater decrease in the density and fractal dimensions of the radial peripapillary capillaries of the inner ONH (p=0.005, rs=0.5; p=0.004, rs=0.6, respectively). A decrease in RNFL thickness was found one year after the start of IVI treatment with brolucizumab (p<0.001).CONCLUSION. According to OCT angiography, vascular perfusion density of the ONH was decreased during IVI of brolucizumab, and RNFL thickness was also decreased one year after the start of treatment.

Abstract: Intravitreal ranibizumab therapy proved effective in exudative age-related macular degeneration with concomitant glaucoma. Timely treatment of both AMD (i.e. before the development of subretinal fibrosis) and glaucoma contributes to therapeutic success in these patients.
