Evaluating recommended audiometric changes to candidacy using the speech intelligibility index

Abstract: Introduction: The National Institute of Health and Care Excellence (NICE) has derived candidacy guidelines for cochlear implants in the UK based on audiometric thresholds (90 dB HL or above at 2 and 4 kHz; hereafter referred to as the 90 dB HL criteria). Recent research has proposed that these criteria should be changed to 80 dB HL at 2 and 4 kHz (hereafter referred to as the 80 dB HL criteria) in the ear to be implanted. Methods: In this study, we analysed aided SII scores derived for different hearing loss p… Show more

“…The unaided SII quantifies the proportion of speech information that is audible to the listener in a given setting. An SII of 0.0 (0%) suggests that no speech information is audible, and an SII of 1.0 (100%) indicates that all speech information in a given setting is audible [ 41 – 45 ]. As SII is often reported as a percentage in clinical applications, we have chosen to report percentages for our findings.…”

“…Whereas the concept of using the aided SII for cochlear implant candidacy has been suggested (Leal, Marriage, & Vickers, 2016), the implementation of the unaided SII as a candidacy criterion for hearing aids is limited by the current lack of evidence about the degree of hearing loss where delays in speech and language begin to emerge across the range of degrees of hearing loss. The goal of this study was Figure 1.…”

“…These guidelines, established in late 2019, suggest that a child who meets 1 of these criteria in at least 1 ear should be referred for evaluation. They include a standard pure tone average (PTA; average at 500, 1000, and 2000 Hz) of 65 dB HL or greater, 22 an aided speech intelligibility index of 0.65 or poorer, [23][24][25][26][27] or an aided word recognition score of 60% or poorer. 17,22 To establish benchmarks for the referral rate of children meeting these criteria, we identified children seen in the past 5 years for diagnostic and hearing aid care across our state-wide healthcare entity who met these newly established criteria.…”
