Background: The potential role of selenium in preventing chronic liver diseases remains controversial. This meta-analysis aimed to summarize the available evidence from observational studies and intervention trials that had evaluated the associations between body selenium status and chronic liver diseases. Methods: We comprehensively searched MEDLINE, Embase, Web of Science, and Cochrane Library from inception to April 2021. The study protocol was registered at PROSPERO (CRD42020210144). Relative risks (RR) for the highest versus the lowest level of selenium and standard mean differences (SMD) with 95% confidence intervals (CI) were pooled using random-effects models. Heterogeneity and publication bias were evaluated using the I2 statistic and Egger’s regression test, respectively. Results: There were 50 studies with 9875 cases and 12975 population controls in the final analysis. Patients with hepatitis (SMD = −1.78, 95% CI: −2.22 to −1.34), liver cirrhosis (SMD = −2.06, 95% CI: −2.48 to −1.63), and liver cancer (SMD = −2.71, 95% CI: −3.31 to −2.11) had significantly lower selenium levels than controls, whereas there was no significant difference in patients with fatty liver diseases (SMD = 1.06, 95% CI: −1.78 to 3.89). Moreover, the meta-analysis showed that a higher selenium level was significantly associated with a 41% decrease in the incidence of significant advanced chronic liver diseases (RR = 0.59, 95% CI: 0.49 to 0.72). Conclusion: Our meta-analysis suggested that both body selenium status and selenium intake were negatively associated with hepatitis, cirrhosis, and liver cancer. However, the associations for fatty liver diseases were conflicting and need to be established in prospective trials.

A cirrose hepática é um processo de cicatrização patológica, irreversível em seus estágios avançados com complicações que podem ser letais. A lesão crônica causa inflamação e fibrose hepática, com consequente formação de septos e nódulos de regeneração fibrosos, que podem levar a complicações futuras como a hipertensão portal. Dentre as suas causas, estão as hepatites virais, hepatite alcoólica, doença hepática gordurosa não alcoólica, doenças autoimunes, colestásticas e de armazenamento. Testes de fibrose não-invasivos são úteis para definir o grau de progressão da doença, com o intuito de intervir e definir a melhor conduta para cada paciente. Terapias antifibróticas, atualmente, visam à inativação da diferenciação das células hepáticas estreladas, responsáveis pela progressão fibrótica. No entanto, ainda se encontram em fase de testes, mas parecem ser promissoras. Trata-se de uma revisão bibliográfica sistemática, de natureza quantitativa, que utilizou as plataformas PubMed (Medline), Scientific Eletronic Library On-line (SciELO) e Cochrane Library como bases de dados para a seleção dos artigos, na língua inglesa. A escolha dos artigos foi realizada por meio da leitura do título, resumo e, por fim, da leitura do artigo na íntegra, sendo realizada uma análise criteriosa fundamentada nos critérios de inclusão e exclusão. Conclui-se, portanto, que as principais etiologias da cirrose hepática são as hepatites virais, hepatite alcóolica e doença hepática gordurosa não alcoólica. Observou-se que as diferentes etiologias causam diferentes formas de evolução fibrótica, que dependem da origem dos tipos celulares e mecanismos envolvidos, fundamentais para avaliar em qual estágio a terapia teria o máximo desempenho.

Introduction Transplantation of IC-2-engineered bone marrow-derived mesenchymal stem cell (BM-MSC) sheets (IC-2 sheets) was previously reported to potentially reduce liver fibrosis. Methods This study prepared IC-2-engineered cell sheets from multiple lots of BM-MSCs and examined the therapeutic effects of these cell sheets on liver fibrosis induced by carbon tetrachloride in mice. The predictive factors for antifibrotic effect on liver fibrosis were tried to identify in advance. Results Secreted matrix metalloproteinase (MMP)-14 was found to be a useful predictive factor to reduce liver fibrosis. Moreover, the cutoff index of MMP-14 for 30% reduction of liver fibrosis was 0.918 fg/cell, judging from univariate analysis and receiver operating curve analysis. In addition, MMP-13 activity and thioredoxin contents in IC-2 sheets were also inversely correlated with hepatic hydroxyproline contents. Finally, IC-2 was also found to promote MMP-14 secretion from BM-MSCs of elderly patients. Surprisingly, the values of secreted MMP-14 from BM-MSCs of elderly patients were much higher than those of young persons. Conclusion The results of this study suggest that the IC-2 sheets would be applicable to clinical use in autologous transplantation for patients with cirrhosis regardless of the patient's age.
