Deliberative ways of dealing with ethical issues in health care are expanding. Moral case deliberation is an example, providing group-wise, structured reflection on dilemmas from practice. Although moral case deliberation is well described in literature, aims and results of moral case deliberation sessions are unknown. This research shows (a) why managers introduce moral case deliberation and (b) what moral case deliberation participants experience as moral case deliberation results. A responsive evaluation was conducted, explicating moral case deliberation experiences by analysing aims (N = 78) and harvest (N = 255). A naturalistic data collection included interviews with managers and evaluation questionnaires of moral case deliberation participants (nurses). From the analysis, moral case deliberation appeals for cooperation, team bonding, critical attitude towards routines and nurses' empowerment. Differences are that managers aim to foster identity of the nursing profession, whereas nurses emphasize learning processes and understanding perspectives. We conclude that moral case deliberation influences team cooperation that cannot be controlled with traditional management tools, but requires time and dialogue. Exchanging aims and harvest between manager and team could result in co-creating (moral) practice in which improvements for daily cooperation result from bringing together perspectives of managers and team members.

Background:In community-engaged research (CEnR), recruitment and data collection are often assigned to community research workers (CRWs). CRWs are hired because of close ties with target communities and presumed greater success recruiting and gathering data from marginalized populations. The value conflicts and personal stress facing CRWs have been described in the international context; however, less attention has been paid to these issues in U.S. settings. Methods: Qualitative interviews were held with CRWs about their experiences with CEnR. Interviews were transcribed and coded using Atlas.ti. Results: The data indicate that CRWs working in settings controlled by the participants, such as neighborhood streets and participant homes, are more likely to face conflicts between following the norms of research and meeting what they perceive to be their everyday obligations to help specific others (duties of relational ethics) and to keep themselves safe. This suggests that "physical proximity" (understood as degree of penetration into the physical domain of the target research community) is an important factor in the ethics of actual practice in CEnR. Conclusions: CRWs often experience ethical complexities that are simply not in the realm of experience of most higher level researchers. How CRWs navigate conflicting ethical obligations can affect the quality and usefulness of the data collected and thereby affect communities by influencing policies informed by these data. Soliciting the perspectives of CRWs early in the process of research design and development is recommended to maximize the benefits achievable through CEnR.

Psychiatric disorders place considerable burden on individuals and on public health. Funding for research in psychiatry is less than ideal, but even so high quality research is being conducted at many centers. However, these studies have not impacted clinical practice as much as expected. The complexity of psychiatric disorders is one of the reasons why we face difficulties in translating research results to patient care. New technologies and improved methodologies are now available and must be incorporated to deal with this complexity and to accelerate the translational process. I discuss the application of modern techniques for data acquisition and analysis and also the new possibilities for performing trials in virtual models of biological systems. Adoption of new technologies is necessary, but will not reduce the importance of some of the fundamentals of all psychiatry research, such as the developmental and translational perspectives. Psychiatrists wishing to integrate these novelties into their research will need to work with contributors with whom they are unaccustomed to working, such as computer experts, a multidisciplinary team, and stakeholders such as patients and caregivers. This process will allow us to further understand and alleviate the suffering and impairment of people with psychiatric disorders. Keywords: Research, trends, psychiatry. ResumoOs transtornos psiquiátricos são responsáveis por uma significativa carga de doença tanto no nível individual quanto na saúde pública. Mesmo com financiamento abaixo do ideal, muitas pesquisas de alta qualidade vêm sendo executadas em vários centros. Entretanto, o impacto desses estudos na prática clínica é menor que o esperado. A complexidade dos transtornos psiquiátricos é uma das razões pelas quais enfrentamos tanta dificuldade na translação dos resultados das pesquisas para a prática clínica. Novas tecnologias e metodologias aperfeiçoadas já estão disponíveis e devem ser incorporadas para lidar com esta complexidade e acelerar o processo translacional. Discuto, neste artigo, a aplicação de técnicas modernas para a coleta e análise de dados e as novas possibilidades para a realização de testes em modelos virtuais dos sistemas biológicos. A adoção das novas tecnologias é necessária, mas não reduzirá a importância de fundamentos da pesquisa em psiquiatria, como as perspectivas desenvolvimental e translacional. Os psiquiatras que desejarem integrar essas novas tecnologias à suas pesquisas terão que trabalhar com colaboradores com os quais não estão acostumados, como especialistas em informática, equipes multidisciplinares e representantes de partes interessadas nos resultados, como pacientes e provedores de cuidados assistenciais. Esse processo permitirá um avanço no conhecimento e no alívio do sofrimento e da incapacidade das pessoas com transtornos psiquiátricos.
