El daño cerebral adquirido (DCA) puede provocar secuelas físicas, motrices, neuropsicológicas y generar una discapacidad en las personas afectadas. La rehabilitación neuropsicológica holística trata de reducir las secuelas cognitivas, conductuales, emocionales y sociales. El abordaje holístico plantea un trabajo que combine intervenciones individuales, grupales, la práctica en entornos lo más reales posibles, así como la intervención familiar y vocacional. Este tipo de rehabilitación busca generalizar los resultados alcanzados a la vida cotidiana, para incrementar la funcionalidad, autonomía y calidad de vida. El objetivo del estudio fue analizar y comparar el rendimiento cognitivo, y la calidad de vida percibida, antes y después de realizar un programa de rehabilitación neuropsicológico holístico con actividades en entornos reales. Se analizó una muestra de 20 personas con DCA. La evaluación neuropsicológica analizó atención, memoria, funciones ejecutivas y calidad de vida. Los resultados mostraron diferencias estadísticamente significativa en el rendimiento de atención, memoria de trabajo, funciones ejecutivas, y calidad de vida. Estos hallazgos sugieren que la rehabilitación neuropsicológica holística, que englobe el trabajo en entornos reales, podría mejorar la generalización de la evolución cognitiva a la vida cotidiana, e incrementar la calidad de vida de las personas con DCA.

Background: Neuropsychological assessments are essential to define the cognitive profile and contribute to the diagnosis of Alzheimer’s disease (AD). The progress in knowledge about the pathophysiological process of the disease has allowed conceptualizing AD through biomarkers as a biological continuum that encompasses different clinical stages. Objective: To explore the association between cerebrospinal fluid (CSF) biomarkers of AD and cognition using the NEURONORMA battery, in a sample of cognitively unimpaired (CU), mild cognitive impaired (MCI), and mild dementia of the Alzheimer type (DAT) subjects, and to characterize the cognitive profiles in MCI subjects classified by A/T/N system. Methods: 42 CU, 35 MCI, and 35 mild DAT were assessed using the NEURONORMA battery. Core AD biomarkers [amyloid-β 42 (Aβ42) peptide, total tau (t-tau), and phosphorylated tau 181 (p-tau181)] proteins were measured in CSF. Correlation coefficients, multivariate regression, and effect sizes were calculated. We explored the age- and education-adjusted cognitive profiles by A/T/N variants within the MCI group. Results: Cognitive outcomes were directly associated with CSF Aβ42 and inversely with CSF tau measures. We found differences in both biomarkers and cognitive outcomes comparing all pairs except for CSF measures between cognitively impaired groups. The highest effect size was in memory tasks and the biomarkers ratios. Lower performances were in memory and executive domains in MCI subjects with AD pathology (A+T+N±) compared to those with normal levels of AD biomarkers (A– T– N). Conclusion: This study provides further evidence of the validity of Spanish NEURONORMA cognitive battery to characterize cognitive impairment in the AD pathological continuum.

Split-half reliability estimates of an online card sorting task in a community sample of young and elderly adults

Executive function is vital for normal social, cognitive, and motor functions. Executive function decline due to aging increases the risk of disability and falls in older adults, which has become an urgent public health issue. Fast and convenient neuropsychological tools are thus needed to identify high-risk groups as early as possible to conduct a timely intervention. Card sorting tasks, such as Wisconsin Card Sorting Task (WCST) and its variants, are popular tools for measuring executive function. This study investigated the reliability of an open-source, self-administered, online, short-version card sorting task with a sample of young ( n = 107, 65 females, age: M = 30.1 years, SD = 5.5 years) and elderly Chinese ( n = 113, 53 females, age: M = 64.0 years, SD = 6.7 years). We developed an automated scoring and visualization procedure following the recent recommendations on scoring perseverative responses to make the results comparable to the standardized WCST. Reliability estimates of commonly used measures were calculated using the split-half method. All task indices' reliabilities were reasonably good in both old and young groups except for "failure-to-maintain-set." Elderly Chinese adults showed compromised task performance on all measures compared with the young Chinese adults at the group level. The R script of automated scoring and estimation of reliability is publicly available.
