Estimation of the Youden Index and its Associated Cutoff Point

Abstract: SummaryThe Youden Index is a frequently used summary measure of the ROC (Receiver Operating Characteristic) curve. It both, measures the effectiveness of a diagnostic marker and enables the selection of an optimal threshold value (cutoff point) for the marker. In this paper we compare several estimation procedures for the Youden Index and its associated cutoff point. These are based on (1) normal assumptions; (2) transformations to normality; (3) the empirical distribution function; (4) kernel smoothing. These… Show more

“…The PLI and SIQR correlated at r ϭ 0.62, with Cronbach's ␣ ϭ 0.95 and 0.89, respectively. These 2 continuous predictors were entered simultaneously into a logistic regression model (evaluated by Wald's statistic) to produce the receiver operating curve (ROC) to determine the optimal threshold for sensitivity and specificity with respect to the 1990 ACR criteria, using the Youden index (sensitivity ϩ specificity Ϫ 1) (17). The Hosmer-Lemeshow test was used to evaluate goodness of (20).…”

“…The Youden index was higher when we used the cut-off of ≤11 instead of 10 (0.285 versus 0.265). The Youden index is a frequently used summary measure of the ROC curve, measuring the effectiveness of a diagnostic marker and enabling the selection of an optimal threshold value for the marker 8 . The index ranges from -1 to +1.…”

“…We considered a decline in FVC of >0.08 L/year on the ROC curve to be the optimal cut-off point by the Youden Index method (26) and then determined the optimal cut-off predictive score to be 0.398290 with the following formula: The sensitivity, specificity, positive predictive value, and negative predictive value of this cut-off value were 75.0%, 85.7%, 75.0%, and 85.7%, respectively.…”
