Using the PRISMA approach, we present the first systematic literature review of digital twin (DT) research in healthcare systems (HSs). This endeavor stems from the pressing need for a thorough analysis of this emerging yet fragmented research area, with the goal of consolidating knowledge to catalyze its growth. Our findings are structured around three research questions aimed at identifying: (i) current research trends, (ii) gaps, and (iii) realization challenges. Current trends indicate global interest and interdisciplinary collaborations to address complex HS challenges. However, existing research predominantly focuses on conceptualization; research on integration, verification, and implementation is nascent. Additionally, we document that a substantial body of papers mislabel their work, often disregarding modeling and twinning methods that are necessary elements of a DT. Furthermore, we provide a non-exhaustive classification of the literature based on two axes: the object (i.e., product or process) and the context (i.e., patient's body, medical procedures, healthcare facilities, and public health). While this is a testament to the diversity of the field, it implies a specific pattern that could be reimagined. We also identify two gaps: (i) considering the human-in-the-loop nature of HSs with a focus on provider decision-making and (ii) implementation research. Lastly, we discuss two challenges for broad-scale implementation of DTs in HSs: improving virtual-to-physical connectivity and data-related issues. In conclusion, this study suggests that DT research could potentially help alleviate the acute shortcomings of HSs that are often manifested in the inability to concurrently improve the quality of care, provider wellbeing, and cost efficiency.

Systems engineers use the term sociotechnical system in academic literature and in their practice. Sociotechnical systems are gaining more attention as systems engineers aspire to address the social elements of their systems engineering practice as well as societal challenges, such as those included in the International Council on Systems Engineering (INCOSE) Vision 2035. Even though there is a basic working definition of sociotechnical system in the Systems Engineering Book of Knowledge (SEBoK), use of the term varies in systems engineering literature depending on application domain. As systems engineering research and practice venture into social domains, it is critical that systems engineers have a shared understanding of terms they use as a foundation of knowledge and practice. To contribute to this foundation, this study is a systematic literature review of how the term sociotechnical system is used in systems engineering literature. We used the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) methodological framework and Web of Science (WoS) for conducting the systematic literature review. We only included peer‐reviewed systems engineering academic papers in this study, and these papers had to either explicitly define the term or implicitly define it by context in the paper. In total, 61 papers were included after inclusion criteria were met, and these were evaluated and synthesized into definition categories. Evaluation and synthesis were conducted according to the PRISMA framework by the study authors in order to manage bias. We found that most papers use sociotechnical system generically as a system that includes social (people) and technical elements. Papers with more refined definitions of sociotechnical system stem from two distinct theoretical traditions: ergonomics/safety and philosophy of engineering. We do not aim to propose a single, normative definition of sociotechnical system. This study is limited by including only systems engineering literature since sociotechnical system has established definitions in other disciplines (e.g., social sciences disciplines). However, the outcome of this study provides systems engineers with documented understanding of how the term sociotechnical system is used within systems engineering.

Prior advances in systems engineering (SE) theory were instrumental in defining the discipline and its tools, but are limited in perspective. The SE community needs new theoretical advances to address its existing and emerging sociotechnical challenges. This communication paper is a product of an NSF/SERC/INCOSE funded workshop on theory building in SE with a focus on the use of abstraction and elaboration. The overarching goals of the workshop were twofold. First, to illustrate the nuances and complexities of the theory building process with an emphasis on developing theory about the SE discipline rather than about a particular system of interest. The second goal was to stimulate a new wave of SE theory by providing a set of frames that could be used for formal theory articulation in follow‐up research. The workshop focused on abstraction and elaboration due to their richness and pervasiveness in SE and investigated the concept in two interrelated contexts: (a) the social coordination of design teams and (b) its strategic use during system design. For each research context, we offer a structured guideline for theorizing, and provide an understanding and framing of some of the generalized abstraction and elaboration uses in SE. Research frames articulated in this workshop could serve as the first step for future research on SE theory, which necessitates hypotheses generation, data collection, validation, and refinement.
