Osteoporosis is a metabolic bone disorder influencing approximately 40% of postmenopausal females, caused by a suppression in bone mass leading to bone weakness and fractures. Herbal medicine commonly used rather than conventional drugs due to its minor adverse effects. Lepidium sativum L. (LS) or "hab arachad", is known as an alternative medication for fracture treatment. The aim of this study is to evaluate the effect LS extract loaded chitosan nanoparticles on gene expression of miR-142-3p and miR-23a in experimental rat model of osteoporosis. The osteoporosis was induced in rats by subcutaneous injection with methylprednisolone (3.5 mg/kg b.wt / day) for 4 weeks. Then osteoporotic rats were orally treated with 400 mg/kg b.wt /day LS extract, chitosan nanoparticles (CN) and 400 mg/kg b.wt /day LS extract loaded on chitosan nanoparticles for 12 weeks. Our results showed that treatment with LS, CN and LS loaded on CN ameliorated the biochemical bone indices with variable degrees, with a final results of improved serum calcium and phosphorus. Treated rats showed a significant decrease in miR-23a expression and a significant increase in miR-142-3p expression which emphasized the improvement of osteoporosis. The effect of treatment with LS and LS loaded on CN was confirmed by histopathological observations. All of these effects together with the safety and no side effects of LS and LS loaded on CN make it a hopeful therapeutic agent for Osteoporosis management.

Background: Osteoporosis (OP) has become an increasingly recognized complication among patients with chronic liver disease (CLD).Objective: This study aimed to determine the prevalence of osteoporosis among patients with CLD and to identify the risk factors associated with osteoporosis in these patients. Materials and Methods: One hundred patients who visited Basrah Gastroenterology and Hepatology Hospital between February 2020 and December 2021 with assessed osteoporosis. Using dual energy X-ray absorptiometry (DEXA), bone density was evaluated. The severity of liver disease, smoking, alcohol use, demographic, biochemical characteristics were all taken while assessing the risk factors for osteoporosis. Results: Of the 100 patients (male/female: 44%/56%, mean age (46 ± 11.5), 50% had CLD diagnoses, 50% did not. Of the 50 patients with CLD,20 patients had OP(40%),while four patients (8%) of the 50 patients without CLD had OP. Twentyfour (24%) of the total patients who undertook the DEXA study showed OP, while seventy-six (76%) did not. Twenty of the twenty-four individuals with OP had CLD (84%) while just four (16%) did not. Child-Pugh A made up 26% of CLD patients, Child-Pugh B 30%, while Child-Pugh C 44%. In this study, osteoporosis and osteopenia were present at rates of 24% and 31% respectively. OP and CLD appeared to be related in a very substantial way (p 0.001). CLD was identified as an independent predictor of OP by multivariate logistic regression analysis (OR (95% CI):9.09(2.18-37.93), p=0.0 Conclusion: It was determined that OP and CLD are significantly correlated. People with CLD were more likely to develop OP. As a result, routine follow-up of these individuals may require biochemical and bone mineral density (BMD) monitoring.

Osteopontin hormone (OPN) is an extracellular matrix protein that is expressed in bone cells such as osteoblasts and osteoclasts and associated with bone turnover and bone mineral density (BMD) in postmenopausal women with osteoporosis. The aim of the study is to investigate serum levels of circulating OPN and its relationship with biochemical parameters and BMD in postmenopausal women with osteoporosis in Iraq. Serum samples from fifty postmenopausal women were selected from patients attending two educational hospitals in Baghdad, which are AL Wasity Educational Hospital and Baghdad Educational Hospital, during the period from November 2018 to March 2019. Twenty five postmenopausal healthy women were included as a control group. The studied subjects’ ages were in the range of 45-65 years. Dual energy X-ray absorptiometry (DEXA) was the device used to measure bone mineral density and diagnose osteoporosis in both groups. Blood samples were collected from each participant for measuring the serum levels of biochemical parameters (P, Alp, Ca, and OPN) The results of the demographic parameters showed a significant (P≤0.05) increase in mean values of age, menopause duration, and duration of productive life in the patients as compared to the control group. Also, strong positive correlations between patients and the control were recorded in age and duration of productive age, with a weak inverse correlation in menopause duration. While a significant (P≤0.05) decrease in the mean value of body mass index (BMI), BMD, and T-score as compared to the control. The correlation in BMI was weak significant positive while in the other two parameters it was weak significant inverse. The results of the present study showed non-significant differences (P ≥0.05) between the patients and control group for serum biochemical parameters. It was also noticed that there was a significant (P≤0.05) increase in the mean value of osteopontin hormone level in the patients as compared to the control, with a weak significant inverse (p≤0.05) correlation.
