ESC Study Group of Sports Cardiology: recommendations for participation in leisure-time physical activity and competitive sports for patients with ischaemic heart disease

Abstract: Individually designed LPA is possible and encouraged in patients with and without established IHD. Competitive sports may be restricted for patients with IHD, depending on the probability of cardiac events and the demands of the sport according to the current classification.

“…13 Therefore, current guidelines recommend that patients in cardiac rehabilitation or secondary prevention programs perform moderate exercise at between 50% and 90% of peak heart rate (HR peak ) 14,15 Our research group, however, is using an exercise model that incorporates aerobic interval exercise training at 85% to 95% of HR peak , not only in healthy individuals, but also in CHD patients. The results demonstrate that such high-intensity exercise is superior to moderate intensity for improving both peak oxygen uptake (VO 2peak ) and cardiac function, and reducing the risk factors associated with cardiovascular disease, as well.…”

“…In contrast, the US Preventive Services Task Force has stated that there is insufficient evidence supporting screening with exercise testing [75]. A PPS flowchart for middle-aged/senior individuals engaged in leisure SEE has been proposed by the European Association of Cardiovascular Prevention and Rehabilitation including [74]: (i) thorough risk assessment with history, physical examination, baseline ECG and risk score assessment within 10yrs using the ESC Systematic Coronary Risk Evaluation (SCORE, based on age, sex, blood pressure, blood cholesterol, and smoking history) [76,77]; followed by ii) maximal exercise testing, but only in individuals with high risk of coronary events.…”

“…A user centred development process is an integral element of any digital health behaviour change intervention. This ensures that the system adheres to both guidelines and recommendations, [13] [14] [15] while maximising the extensive input from patients and stakeholders. In order to extract feedback from the target user group, a user need analysis was performed, aiming at gathering both quantitative and qualitative information.…”
