Clinical correlates of comorbid attention deficit hyperactivity disorder in adults suffering from bipolar disorder: A meta-analysis

Objective: Attention deficit hyperactivity disorder is a frequent comorbid condition in adults with bipolar disorder. We performed a meta-analysis aimed at assessing sociodemographic and clinical correlates of attention deficit hyperactivity disorder in bipolar disorder. Method: We searched main electronic databases up to June 2021. Random-effects meta-analyses, with relevant meta-regression and quality-based sensitivity analyses, were carried out to estimate the association between attention deficit hyperactivity disorder and putative correlates, grading the quality of evidence. Results: We included 43 studies, based on 38 independent samples. Attention deficit hyperactivity disorder participants were more likely to be males (odds ratio = 1.46; p < 0.001) and unemployed (odds ratio = 1.45; p = 0.045), and less likely to be married (odds ratio = 0.62; p = 0.014). They had an earlier onset of bipolar disorder (standardized mean difference = −0.36; p < 0.001); more mood episodes (standardized mean difference = 0.35; p = 0.007), particularly depressive (standardized mean difference = 0.30; p = 0.011) and mixed (standardized mean difference = 0.30; p = 0.031) ones; higher odds of using antidepressants (odds ratio = 1.80; p = 0.024) and attempted suicides (odds ratio = 1.83; p < 0.001) and lower odds of psychotic features (odds ratio = 0.63; p = 0.010). Moreover, they were more likely to have generalized anxiety disorder (odds ratio = 1.50; p = 0.019), panic disorder (odds ratio = 1.89; p < 0.001), social phobia (odds ratio = 1.61; p = 0.017), eating disorders (odds ratio = 1.91; p = 0.007), antisocial personality disorder (odds ratio = 3.59; p = 0.004) and substance (odds ratio = 2.29; p < 0.001) or alcohol (odds ratio = 2.28; p < 0.001) use disorders. Quality of the evidence was generally low or very low for the majority of correlates, except for bipolar disorder onset and alcohol/substance use disorders (high), and suicide attempts (moderate). Conclusion: Comorbid bipolar disorder/attention deficit hyperactivity disorder may have some distinctive clinical features including an earlier onset of bipolar disorder and higher comorbid alcohol/substance use disorder rates. Further research is needed to identify additional clinical characteristics of this comorbidity.

ÖzetTürkiye'de bipolar bozukluk ile ilgili yapılmış çalışmaların incelenmesinde epidemiyolojik anlamda nitelikli alan çalışmasına rastlanmamıştır. Çalışmaların çoğu, hastane başvurusu kaynaklı ve klinik çalışmalardır, sistematik bir tarama içermemekte, epidemiyolojik veri toplama ya da sağlık hizmetlerine bilgi aktarma amaçlı değildir. Bu çalışmaların bulgularının toplumumuza genellenebilirliği geçerli ve güvenilir olmaktan uzaktır. Bununla birlikte, ülkemizin değişik bölgelerinde yapılmış çalışmaların bulgularının gözden geçirilerek oluşturulacak bir profilin hastalığın bize özgü özellikle-rinin tanınmasına ve risk etkenlerinin saptanmasına katkısı olacaktır. Tüm bu bilgiler bipolar bozukluk ile ilgili olarak, toplum ruh sağlığı hizmetlerinin oluşturulup geliştirilmesine temel oluştu-rabilir. Ülkemizde epidemiyolojik anlamlılığı olan, çok merkezli, toplum örneklemli ve geniş katılım-lı çalışmalara gereksinimi vardır. Psikiyatrik epidemiyoloji ve klinik araştırmalarımız gerekli bilimsel bazı bulundurmakla birlikte, hızlı kentleşme ve göç gibi toplumsal, özgün ve gelişimsel ilişkileri değerlendirebilecek güçte değildir. Bu gözden geçirmede, Türkiye'de bipolar bozukluk ile ilgili yapılmış çalışmalarda, öne çıkan epidemiyolojik bulguların, eksikliklerin ve gelecekte konuyla ilgili neler yapılabileceğinin araştırılarak tartışılması amaçlanmıştır.Anahtar sözcükler: Bipolar bozukluk, epidemiyoloji, Türkiye. AbstractAssessment of studies related with bipolar disorder in Turkey did not reveal an epidemiologically qualified field study. Most of the studies are hospital based or clinical studies which did not include a systematical scanning and did not aim to collect any epidemiological data and transfer information to health authorities. The generalizability of these studies to our community is far from being valid and reliable. On the other hand, a profile which will be created by reviewing the findings of these studies performed in various regions of Turkey will have a contribution to knowing the "unique to us" features of bipolar disorder and determining cultural risk factors. All this information can constitute a basis for formation and development of public mental health services related with bipolar disorder. In Turkey, the need for epidemiologically significant, polycentric, public sampled studies with broad participation is indispensable. Although our psychiatric epidemiology and clinical studies include necessary scientific basis, they are not powerful enough to evaluate the authentic and progressional relations such as rapid urbanization and immigration. The aim of this review is to evaluate and discuss prominent epidemiological findings, deficiencies and possibile future activities related with studies conducted in Turkey about bipolar disorder..

INTRODUCTION:Since extended-release formulations of CBZ have been developed to decrease daily fluctuations in serum CBZ concentrations and improve dosing convenience, several large clinical trials have recently been conducted to assess the efficacy and tolerability of this form of CBZ in bipolar disorder. In the present assessment, extendedrelease carbamazepine has been compared with lithium, in a head-to-head evaluation, to assess its efficacy and safety in a group of non-Western patient population with diagnosis of acute mania. METHOD: Fifty male inpatients with diagnosis of bipolar I disorder, according to Diagnostic and statistical manual of mental disorders, 5th edition (DSM-5), entered into a 3-week study, for random assignment to extended-release carbamazepine or lithium carbonate. The assessment had been accomplished as a double-blind design, while the patients, staff, prescribers, and assessor were unaware of the prescribed drugs that were packed into identical capsules. Primary outcome measure in the present assessment was Manic State Rating Scale (MSRS), which had been scored at baseline and weekly intervals up to the third week. Also, insight and overall illness severity and improvement had been rated by the Schedule for Assessment of Insight (SAI), Clinical Global Impressions-Global Improvement Scale (CGI-I), and Clinical Global Impressions-Severity Scale (CGI-S), respectively. Treatment effectiveness had been analysed by t-test and repeated measures analysis of variance (ANOVA). Statistical significance had been defined as p-value ≤.05. RESULTS: While mean total score of MSRS improved significantly by both lithium and extended-release carbamazepine at the end of the third week, between-group analysis displayed significant advantage of lithium, regarding both frequency and intensity, at the end of trial. Mean total score of SAI showed significant improvement by both of them. However, again, the CGI-I demonstrated significant improvement by extended-release carbamazepine and lithium, CGI-S revealed significant progress only by lithium. Moreover, the effect size (ES) analysis showed large improvement of MSRS with lithium and medium improvement by extended-release carbamazepine. Post hoc power analysis showed an intermediate power of 0.42 on behalf of the present assessment. CONCLUSION: Although both extended-release carbamazepine and lithium were helpful for improvement of manic symptoms, treatment with lithium seems to be more advantageous. ARTICLE HISTORY
