1. The sensitivity of the HSS Knee Score is insufficient if tests are carried out at short intervals. It does not provide a balanced assessment of the functional capability and clinical performance of the joint. 2. The modified Staffelstein-Score has a high sensitivity to clinical changes, even those occurring a few days following an arthroplasty procedure. 3. The MSS is also useful as a balanced assessment of pain, joint functional capability and clinical examination results.

▼Objective: The success of an individually optimized physiotherapy after implantation of a knee endoprothesis depends strongly on postoperative pain of the patient. Therefore, the task of every post-operative care is to reduce this pain. In this study, the effi cacy of medium frequency current to the course of post-operative pain and other parameters is tested. Method: In a single-blinded randomized clinical study, 30 successive patients undergoing implantation of a knee endoprothesis were included; after surgery, 15 got electrotherapy with the IONOSON-Expert ® additionally to the physiotherapy, the other 15 served as controls and had only physiotherapy according to our optimized scheme. To prove a possible superiority of the combined electro and physiotherapy treatment against physiotherapy only, we choose the pain score as the primary target variable. Some isokinetic parameters like the ‚angle of lower leg movement' and the physical work were additionally evaluated (LIDO Active ® , LOREDAN BIOMEDI-CAL). Results: None of the parameters showed a statistically signifi cant improvement in the group with additional electrotherapy against the control group. However, we could show a signifi cant correlation between the reduction of pain and the increase of the angle of lower leg movement. A tendency towards an increased reduction of pain in the electro-therapy group compared to the control group could be seen, however, this improvement was not statistically signifi cant. Conclusion:The results of our pilot study provide the basis for a sample size calculation for a subsequent clinical study to prove the statistical signifi cance of postoperative pain reduction using a medium frequency therapy and of an increased range of movement. Zusammenfassung ▼Hintergrund: Das Ausmaß des Schmerzes nach Knie-Endoprothesen-Operationen bestimmt sehr wesentlich den Verlauf der postoperativen Therapiemaßnahmen und somit auch den Erfolg der Nachbehandlung bzw. der Rehabilitation. Schmerzreduktion ist daher ein zentrales Thema der postoperativen Behandlung. Dies kann unter anderem auch mit Elektrotherapie erfolgen. In der vorliegenden Studie soll die Wirksamkeit eines amplitudenmodulierten mittelfrequenten Wechselstroms (AMS) auf den Behandlungsverlauf überprüft werden. Methode: In einer einfach verblindeten randomisierten Pilotstudie wurden von 30 Patienten, die eine Knie-Endoprothese bekommen hatten, 15 Patienten einer Elektrotherapie mit dem IO-NOSON Expert ® unterzogen, während sie mit dem Klinikschema entsprechender Physiotherapie behandelt wurden. Die anderen 15 Patienten bildeten die Kontrollgruppe; sie wurden wäh-rend der physiotherapeutischen Behandlung zwar an den IONOSON Expert ® angeschlossen, das Gerät wurde jedoch nicht angeschaltet. Zur statistischen Prüfung einer möglichen Wirkung des AMS wurde als wichtigstes Kriterium der Schmerzscore (visuelle Analogskala -VAS) gewählt. Cell-Stimulation Therapy of Lateral Epicondylitis with Frequency-Modulated Low-Intensity Electric Current

In addition to the routine therapy, the patients with lateral epicondylitis included into experimental group were subjected to a 12-week cell-stimulation therapy with low-intensity frequency-modulated electric current. The control group received the same routine therapy and sham stimulation (the therapeutic apparatus was not energized). The efficiency of this microcurrent therapy was estimated by comparing medical indices before therapy and at the end of a 12-week therapeutic course using a 10-point pain severity numeric rating scale (NRS) and Roles-Maudsley pain score. The study revealed high therapeutic efficiency of cell-stimulation with low-intensity electric current resulting probably from up-regulation of intracellular transmitters, interleukins, and prostaglandins playing the key role in the regulation of inflammation.
