BackgroundCerebral palsy (CP) is the most frequent cause of motor handicap among children.Aim of the studyWe aim to study the relation of epilepsy in children with CP to various risk factors that affect the development of seizures.Patients and methodsIn a cross-sectional, descriptive, population-based, case-control study, 98 children with CP (48 children with CP with epilepsy, and 50 children with CP without epilepsy) were compared with 180 children without CP or seizures. The children lived in two regions in Egypt: the Al-Kharga District–New Valley and El-Quseir city–Red Sea. These cases were subjected to meticulous neurological assessment, brain magnetic resonance imaging, electroencephalography, and Stanford-Binet (4th edition) examination. Multinomial logistic regression was used to assess the risk factors.ResultsEpilepsy was diagnosed in 48.9% of all cases of CP. Mental subnormality was observed more frequently in children with epilepsy than in those without epilepsy (84.6% versus 66.7%). The frequency of epilepsy was highest in patients with the spastic quadriplegic type of illness (58.3%). Multinomial logistic regression revealed that prematurity (<32 weeks of pregnancy), low birth weight (<2.5 kg at birth), neonatal seizures, jaundice, and cyanosis were significantly associated with CP with epilepsy.ConclusionCP is associated with a high percentage of seizure disorders. Prematurity, low birth weight, neonatal seizures, cyanosis, and jaundice are significant risk factors among patients with CP with epilepsy compared to patients with CP without epilepsy or a healthy control group.

Epilepsy is one of the most common neurologic disorders in childhood that often requires long term treatment with antiepileptic drugs. Both antiepileptic treatment and the comorbidities associated with epilepsy have a negative impact on bone health in growing children. Given the fact that vitamin D deficiency is a major public health problem worldwide, clinicians caring for children with chronic diseases should be aware of effects of the medication on the bone metabolism. Yet, vitamin D deficiency due to antiepileptic treatment is an overlooked issue among neurologists. In this review, we briefly describe vitamin D metabolism and the effect of vitamin D in the brain. We also discuss the literature in terms of vitamin D deficiency and antiepileptic treatment in the pediatric population.

ÖzetAmaç: Çalışmamız rehabilitasyon merkezlerine yönlendirilen nörogelişimsel bozukluğu olan çocukların durumlarını ortaya koymak ve risk etmenlerini belirlemek amacıyla planlandı. Gereç ve Yöntem: Engelli çocuklar rehabilitasyon merkezine Nisan 2000 ile Nisan 2012 yılları arasında özel eğitim, fizyoterapi ve rehabilitasyon hizmeti almak üzere yönlendirilen toplam 396 nörogelişimsel bozukluğu olan çocuk ve ailesi dahil edildi. Çocukların dosyaları geriye dönük olarak incelendi. Bulgular: Çocukların %52,8'i (n=209) erkek, %47,2'si (n=187) kızdır. Başvuru yapan en küçük çocuğun yaşının bir ay sekiz günlük en büyüğün ise 32 yıl beş ay dokuz günlük olduğu saptandı. Nörogelişimsel bozukluğu olan çocuklar içerisinde beyin felci ilk sırada, zeka geriliği olan çocuklar ikinci sırada yer aldı. Normal servikovajinal yol veya sezaryen ile yapılan doğumlarda kuadriparetik beyin felci tipinin en fazla olduğu bulundu. Gebelik yaşı ile beyin felci tipi arasında fark bulunmadı (p<,05). Beyin felci tipinin doğum şekline göre değişiklik gösterdiği bulundu (z=-3,021, p=0,003). Annelerin %32,8'i çalışırken, babaların %73,5'i çalışmaya devam etmekteydi. Annelerin hamilelik dönemi boyunca %92,2'sinin düzenli doktor kontrolüne gittikleri, %97'sinin sağlık kuruluşunda doğum yaptıkları ve %51'inin normal doğum yaparken %49'unun sezaryen ile doğum yaptığı saptandı. Çıkarımlar: Çalışmamız ülkemizdeki rehabilitasyon merkezlerine yönlendirilen nörogelişimsel bozukluğu olan çocukların durumlarını ortaya koyan ilk ve tek çalışma özelliğini taşımaktadır. Bu çalışmanın sonuçları merkezlere yönlendirilen çocuk ve ailelerin gereksinimlerinin saptanmasına, eğitim, fizyoterapi ve rehabilitasyon yaklaşımlarının kapsam ve içeriğinin bu doğrultuda belirlenmesine katkı sağlayacaktır. Material and Method: Three hundred and ninety-six children and their parents who were referred to a rehabilitation clinic for special education, physiotherapy and rehabilitation between the years April 2000 and April 2012 were included in this study. All their records were analyzed retrospectively. Results: The 52.8% (n=209) of the children were boys and 47.2% (n=187) were girls. Number of children with Cerebral Palsy (CP) were in the first raw and children with Mental Retardation (MR) were the second in children with neurodevelopment disabilities. Quadriparetic type of CP was the most both in spontaneous cervicovaginal and caesarian deliveries. There were no statistical difference between the gestational age and types of CP (p<.05).
