O cisto epidérmico (C.E.), também conhecido como cisto sebáceo, é um nódulo semelhante a um caroço, da mesma cor da pele, esbranquiçado, ou amarelado, que tem tamanho variável e pode ser único ou múltiplo, com consistência dura, elástica, com flutuação ou pus quando inflamado. Acometem os pacientes de qualquer idade, são benignos e mais comuns na face, pescoço e tronco, embora possam acometer qualquer região do corpo. Ocasionalmente, provocam dor e têm coloração avermelhada. O objetivo desse trabalho é relatar um caso clínico de um paciente que procurou o serviço de Cirurgia e traumatologia Buco-maxilo-facial do Hospital da Restauração, Recife-PE, com história de aumento de volume em face de característica cística após trauma local, discutindo, assim, o diagnóstico a partir de exames imaginológicos e histopatológicos e seu tratamento cirúrgico. O C.E pode representar um grande risco à saúde do paciente, uma vez que pode gerar efeitos significativamente deletérios, que vão desde defeitos estéticos de grandes proporções até obstrução de vias aéreas, que pode fomentar o óbito, se não manejado corretamente. Com isso, é necessário enfatizar a necessidade do conhecimento do cirurgião, para que desta forma o diagnóstico e tratamento sejam efetivos e o paciente seja minimamente afetado pelas morbidades desta entidade patológica.

BackgroundEpidermoid cysts are benign lesions encountered throughout the body. Eighty percent of epidermoid cyst seen in ovaries and testicles, whereas in head and neck region they account for only 1.6–7.0%. 1.6% of epidermoid cysts occur in oral cavity and they account for 0.01% of all the oral cavity cysts.Case DetailsTwo case reports of epidermoid cyst has been discussed. One patient reported with well defined swelling in the right lower one third of the face, another one in the upper lip. Initially the cases were diagnosed as Lipoma and salivary adenoma respectively but histologically they turned out to be epidermoid cyst. Hence, we aim at highlighting the differential diagnosis pertaining to the anatomical location. The PubMed database search made on January 2016 yielded 674 articles of epidermoid cyst in the head and neck region. In that there is only one case reported occuring in the lower one third of face. In the current report, we outline the second case of epidermoid cyst localized to the right mandibular body region without bone involvement. Another case in our report was localized in the upper lip. In the literature totally seven cases were reported till now occurring in the lip.ConclusionAlthough epidermoid cysts are rarely encountered in the oral cavity, the possibility that they may occur warrants the need for successful management to avoid misdiagnosis.

An epidermoid cyst is one of the most common benign cysts of the skin. The cyst is filled with keratin flakes and its wall is composed of keratinized, stratified, squamous epithelium. Epidermoid cysts can occur anywhere on the body, usually along the lines of embryonic fusion and mostly on the face, scalp, neck, chest, and upper back. Around 80% of the epidermoid cysts are seen in ovaries and testicles whereas, in the head and neck, they account for only about 7% and 1.6% of epidermoid cysts are reported in the oral cavity. The condition can be congenital or acquired. Congenital causes are rare and may be caused by the entrapment of ectodermal elements intradermally or subcutaneously during embryogenesis. Acquired causes of an epidermoid cyst may result from traumatic or iatrogenic implantation of epithelial cells into the dermal layer or from obstruction of a pilosebaceous unit in the hair follicle. Surgical excision completes the treatment.
