Objective: Describe the clinical and epidemiological profile of patients undergoing total hip arthroplasty by analyzing the correlations between gender and indication of surgery and postoperative complications. Methods:Cross-sectional study in retrospective approach of the medical records of patients undergoing total hip arthroplasty in a private Hospital in Salvador, Bahia in 2013 and 2014.Results: 66, 3% of patients are women, aged between 71 and 80 years (31, 2%), systemic arterial hypertension patients (63, 9%), whose major indication occurred coxarthrosis (48, 2%) followed by hip fracture (39, 8%); mostly by falling from height (66, 7%). Among the complications, anemia prevailed (53%) and death occurred in only 2, 4%. By correlating sex with indication of arthroplasty, hip fracture was only statistically significant in women (p=0.004); in complications, anemia (p=0.024) and urinary tract infection (p=0.025) had statistically significance in women and in men genitourinary changes (p<0.001). Conclusions:Patients undergoing hip arthroplasty are elderly, female, hypertensive, most of whose procedures ran from coxarthrosis or hip fracture. Among the complications prevailed anemia, genitourinary, cardiovascular and digestive changes. Hip fracture, anemia and urinary tract infection are significantly more prevalent in females, while the genitourinary changes are significant in males.

In this study, we examined the effect of variations in nurse staffing levels on the length of stay and medical expenses of patients who underwent hip or knee surgeries. A cross-sectional study was conducted using the National Health Insurance database and hospital surveys from 2010. Patient length of stay and medical expenses by nurse staffing level and skill mix were compared after adjusting for hospital and patient characteristics. Nurse staffing was measured based on staffing grade, the bed-to-registered nurse/nurse aid ratio, the bed-to-nursing personnel ratio, and the RN proportion. Generalized estimation models were used to analyze the associations. Decreased nurse staffing was consistently associated with increased length of stay, regardless of nurse staffing measures. The medical expenses associated with the lowest staffing level were approximately $US 1142.2 more than those associated with the highest staffing level. The study results suggest that maintaining a high nurse staffing level could be a cost-effective strategy for government and insurers, as well as for patients. We propose that policy makers implement more efficient nurse staffing strategies.

Introduction. The matter of the replacement of acetabular defects is becoming increasingly relevant today. In their clinic the authors used custom-made implants with trabecular coating for replacement of defects in the acetabulum where and when it was necessary to ensure extended fixation of the component. Purpose — to evaluate short-term clinical and functional outcomes, osteointegration of porous coated custom-made implants, the overall survival of structural components and to identify the causes of failures and complications. Materials and Methods. During the study the authors interviewed and evaluated radiographs of 48 patients operated from October 2015 to June 2018. The patients were interviewed before and after the surgery using the Oxford hip joint scale, quality of life EQ-5D and visual analogue pain scale. Radiographs were evaluated for a period of at least 12 months for the signs of osteointegration. Results. The average OHS scores increased from 14.9 (±7) to 37.6 (±7) (p<0.01). Quality of life according to the EQ-5D index increased from 0.2 (±0.2) to 0.7 (±0.2) (p<0.01). The average value by general health scales: before operations — 49 (±17) and 73 (±18) scores after surgeries (p<0.01). The average VAS pain score decreased from 73 (±10) to 19 (±19) points (p<0.01). Osteointegration was observed in 98 percent of cases. Migration of the implant with fracture of a flange was observed in one case. Conclusions. In the short-term follow up the use of custom-made implants significantly improves hip joint function and the patients’ life quality. Custom-made implants enable primary reliable fixation in case of complex acetabular defects. High rate of osteointegration is observed for custom-made implants with porous coating within at least 12 months. Longer follow-up is needed to evaluate long-term results.
