Chronic kidney disease (CKD) represents a global public health burden, but its true prevalence is not fully characterized in the majority of countries. We studied the CKD prevalence in adult users of the primary, secondary and tertiary healthcare units of an integrated health region of the north of Portugal (n = 136 993; representing approximately 90% of the region adult population). Of them, 45 983 (33.6%) had at least two eGFR assessments and 30 534 (22.2%) had at least two urinary albumin creatinine ratio (UACR) assessments separated at least 3 months. CKD was defined according to Kidney Disease: Improving Global Outcomes (KDIGO) guidelines as a persistent decrease in eGFR (<60 ml/min/1.73 m2) and/or increase in UACR (≥30 mg/g). The overall prevalence of CKD estimated was 9.8% and was higher in females (5.5%) than males (4.2%). From these, 4.7% were possible to stratified according to KDIGO guidelines. The prevalence of CKD was higher in older patients (specially in patients with more than 70-years-old) and in patients with comorbidities. This is the first real-world based study to characterize CKD prevalence in a large, unselected Portuguese population. It probably provides the nearest estimate of the true CKD prevalence and may help healthcare providers to guide CKD-related policies and strategies focused on prevention and on the improvement of cardiovascular disease and other outcomes.

Objective To assess the efficiency of Dyevert™ Power XT compared to the standard clinical practice when used for percutaneous coronary interventions (PCI). Methods A Markov model was developed to estimate, over 3‐month cycles and a lifetime time horizon, the cumulative costs and health outcomes (life years gained [LYG] and quality‐adjusted life years [QALY]) in a hypothetical cohort of 1,000 patients with chronic kidney disease (CKD) 3b‐4 and an average age of 72 years. The incidence of contrast‐induced acute kidney injury for these patients is 18.89% in routine practice and 7.78% with Dyevert. QALYs were estimated by applying utilities by health state. Transitions between states and utilities were obtained from the literature. Overall all‐cause and state‐specific mortality were considered. The total cost (€2,022) estimated with the National Health System perspective included cost of the procedure and of CKD management. The parameters were validated by a panel of experts. A discount rate (3% per year) was applied to costs and outcomes. Results The use of Dyevert yielded more health benefits (34.60 LYG and 5.69 QALYs) compared to the current standard practice (33.11 LYG and 5.38 QALYs). Lifetime cost accumulated at the end of the simulation resulted €30,211/patient with Dyevert and €33,895/patient with current standard clinical practice. Conclusions The use of Dyevert™ Power XT resulted dominant option, due to its higher effectiveness and lower cost as compared to standard clinical practice and, therefore, a preferred option in patients with CKD stages 3b‐4 undergoing PCI in Spain.

Aspectos fundamentales de la Acidosis metabólica -Revisión bibliográfica 240 RESUMEN Introducción: Es muy importante tener en cuenta que existen enfermedades que tiene que ver con la alteración del pH en el organismo, como es el caso de la acidosis metabólica, que se puede definir como una afección o una patología que se desarrolla en las personas cuando existe un nivel muy alto de ácido en el cuerpo. Objetivo: Describir los aspectos fundamentales de la acidosis metabólica mediante una revisión bibliográfica que permita con disponer de información detallada sobre esta condición al alcance del lector. Metodología: Para la presente revisión bibliográfica, se utilizó la metodología basada en la búsqueda y selección de artículos y libros que contemplen los aspectos fundamentales de la acidosis metabólica. Conclusiones: La acidosis metabólica se desarrolla cuando hay alteraciones a nivel fisiológico que reducen la concentración plasmática de bicarbonato normal (22 mEq/l), la disminución del pH sanguíneo, o la caída de los niveles normales de bicarbonato (HCO3−). Se pueden identificar causas a nivel de la producción excesiva de ácido como en los casos de lactoacidosis por anemia o cirrosis; pérdida excesiva de bases por diarrea, desnutrición y demás o por regeneración insuficiente de bases como sucede en la hiperpotasemia, enfermedad de Addison o por fármacos como la heparina.
