Protein kinase B (PKB/AKT) has shown a high profile in the research of metabolic diseases. This research sought to determine whether the AKT1 gene's single nucleotide polymorphisms (SNPs) and the risk of developing non-alcoholic fatty liver disease (NAFLD) were related. Patients and Methods: Recruited in this case-control study were 2693 subjects, including 815 with NAFLD and 1878 without NAFLD. Three SNPs of AKT1 (rs2494732, rs2494752 and rs1130233) were genotyped. To examine the correlation between SNPs and NAFLD susceptibility, logistic regression was performed. Results: After adjusting for sex, age, triglyceride and glucose, AKT1 rs2494732-C (all P < 0.05 in co-dominant model, dominant model and additive model) and rs2494752-G (P < 0.05 in co-dominant model) were linked to a lower risk of NAFLD. The combined effect of both SNPs on NAFLD risk was statistically significant, showing a dose dependence (P trend = 0.010). Sex, body mass index, hypertension, hyperglycemia, hypertriglyceridemia, high-density lipoprotein-cholesterol, alanine aminotransferase, and beneficial alleles were all significant predictors of NAFLD risk (all P < 0.05). The prediction model achieved good discrimination, with an area under the receiver operating characteristic curve of 0.779. The Hosmer-Lemeshow test suggested an inadequate calibration of the model (χ 2 = 21.073, P = 0.007). Conclusion: AKT1 rs2494732 and rs2494752 may be related to Chinese NAFLD susceptibility. The prediction model combining both SNPs with clinical factors displays a strong ability to discriminate NAFLD patients. Both SNPs may be exploited to design new models for early screening of NAFLD high-risk population.

Purine adenosine pathway exists widely in the body metabolism, and is involved in regulating various physiological processes. It is one of the important pathways of environmental regulation in human body. CD73 is essentially a protease that catalyzes further dephosphorylation of extracellular adenine nucleotides, hydrolyzing extracellular AMP to adenosine and phosphate. CD73 is an important part of the adenosine signaling pathway. Studies have shown that CD73-mediated adenosine pathway can convert the inflammatory ATP into the immunosuppressant adenosine. This paper aims to summarize the relevant effects of CD73 in the occurrence, development and prognosis of liver diseases such as viral hepatitis, highlight the important role of CD73 in liver diseases, especially in viral hepatitis such as HBV and HCV, and explore new clinical ideas for future treatment targets of liver diseases.

Background Metabolic dysfunction-associated fatty liver disease (MAFLD) has been recognized as an accurate reflection of the pathogenesis of fatty liver disease by a consensus document. However, the relationship between MAFLD and the prognosis of patients with hepatocellular carcinoma (HCC) remains unclear. Therefore, the main purpose of this study was to investigate the impact of MAFLD on the prognosis of HCC patients after hepatectomy. Methods To identify risk factors associated with poor prognosis, we used a multivariate Cox proportional hazards model. We also conducted propensity score matching (PSM) analysis to balance the baseline characteristics between the MAFLD and non-MAFLD groups. Kaplan‒Meier survival curves were used to compare the prognosis of the two matched groups. Results This study involved 909 HCC patients who underwent hepatectomy between January 2011 and December 2017 at the West China Hospital of Sichuan University, including 97 patients with MAFLD (MAFLD group) and 812 patients without MAFLD (non-MAFLD group). Our multivariate analysis results indicated that MAFLD might be a protective factor for overall survival (OS) (p = 0.032), but not for recurrence-free survival (RFS) (p = 0.214). After conducting PSM analysis, we found that the RFS rates in the MAFLD group were higher at the 1-, 3-, and 5-year intervals, with rates of 72.7%, 59.0%, and 49.9%, respectively, than those in the non-MAFLD group, which had rates of 69.8%, 54.3%, and 43.2%, respectively. Therefore, MAFLD might improve the 1-year, 3-year, and 5-year RFS rates of HCC patients who undergo hepatectomy. In the subgroup analysis, we found that patients in the overweight group had better RFS than those in the lean/normal group (p＜0.001). Conclusions In summary, our study suggests that MAFLD might be a protective factor for the OS of HCC patients who underwent hepatectomy through multivariate analysis. Furthermore, after PSM analysis, MAFLD was found to improve the 1-year, 3-year, and 5-year RFS rates of HCC patients who underwent hepatectomy, especially those who were overweight. These findings may have implications for the management and treatment of HCC patients with MAFLD.
