Background: It is scarce in the literature studies regarding treatment and objective analysis of the survival chances of burned patients. Some investigations indicates the need for obtaining specific data to the population studied and characteristics such as social condition and nutritional status are suggested to be relevant to the patient's evolution. We aimed to describe the mortality of a Treatment Center Burn Unit (BU) in Brazil. Methods:We analyzed 76 burned victims hospitalized with a fatal outcome in the General Hospital of São Matheus in 10 years (1999)(2000)(2001)(2002)(2003)(2004)(2005)(2006)(2007)(2008)(2009). The following collected data were investigated were: age, sex, period of permanence, body surface burned (BSB), degree of burns, the causal agent, inhalation injury, nature of the event, clinical complications, mortality and survival rates. Results:We observed predominantly male and the median age was 44 years old. The highest incidence was on June. The mean body surface area burned was 40%, and upper limbs, the most affected region. The causative agents involved were more flammable agents and fire. Pulmonary infection was the most frequent clinical complication. The mortality found in this center was 4.9%. The subgroup of suicide were predominantly female and the mortality rate was 22.44%. When studying survival rate, patients with inhalation injury and trauma were associated with shorter survival. Patients with sepsis remained alive for longer. Conclusion:The clinical, epidemiological and survival of patients with burns, enables the design of peculiar characteristics of this trauma in the studied area, thus, from these data, charts be established treatment and prognostic estimates, and assist in developing preventive public health policy more effective.

Burns are devastating injuries that disproportionately affect people in developing countries, including children. In addition to a high mortality rate, survivors are burdened with life-long physical and emotional scars. The etiology and nature of burn injuries varies significantly by country, and this chapter explores the predominant causes and patterns of burn injury in both the developing and industrialized worlds. Gender differences play a significant role in the risk of burn injuries, across a spectrum with a predominance of women injured in fires from cooking and heating fuels in the developing world and industrial accidents primarily affecting men in developed nations. Children are particularly vulnerable to burn injuries, accounting for almost 50% of all burn patients in some studies. A majority of pediatric burns are scald injuries usually affecting very young children below the age of 5 years, and we discuss the behavioral patterns underlying this finding. Finally, the elderly form a rapidly increasing proportion of the population in many countries, and are often burdened with comorbidities that are likely to pose significant challenges in burn care.

ResumoGrande número de casos de queimaduras com álcool entre adultos e crianças é frequentemente apontado nas casuísticas nacionais. A Resolução da ANVISA (Agência Nacional de Vigilância Sanitária) de 2002, que restringia a comercialização de álcool em grandes concentrações para uso doméstico, foi o reconhecimento oficial da existência deste tipo de queimadura como um problema social e de saúde pública. Dias antes de findar o prazo de adequação à medida, os maiores fabricantes do produto foram liberados de a acatarem. Passados alguns anos, buscou-se, na presente tese de doutorado, apreciar quesitos surgidos no debate de então, os quais ainda permanecem, dada sua relevância. Para melhor evidenciar os contornos e as implicações da questão abordada pela iniciativa governamental, este trabalho teve como objetivos: caracterizar a magnitude do problema das queimaduras no Brasil através da análise crítica das informações disponíveis e reconhecer os tipos mais frequentes da lesão; descrever a casuística de um centro brasileiro de referência no tratamento de queimaduras e situá-la em relação ao contexto nacional e internacional; caracterizar cenários e circunstâncias em que crianças foram vítimas de queimaduras com álcool, dentro de uma população delimitada; interpretar, instruídos pelo conceito de vulnerabilidade, as relações dinâmicas e as combinações existentes entre as condições individuais e sociais presentes nos contextos em que crianças sofrem queimaduras com álcool. A investigação empírica aliou ao método epidemiológico descritivo o enfoque qualitativo, apoiado em entrevistas em profundidade a partir de roteiro temático. Os dados permitiram sustentar que o Brasil exibe em seu perfil epidemiológico características que não são encontradas na literatura produzida em outras partes do mundo: alta proporção de queimaduras com álcool entre pacientes hospitalizados, sejam adultos ou crianças. Nas histórias relatadas pelos pais entrevistados foram reconhecidos elementos do universo simbólico das camadas populares, assim como dinâmicas sociais e culturais subjacentes aos comportamentos aparentemente "inadequados" (ou vulneráveis, pela perspectiva adotada na pesquisa) na utilização do álcool, que resultaram na queimadura de seus filhos.
