The majority of sexually active individuals becomes infected with human papillomavirus (HPV) at least once in their lifetime. Pathways for HPV transmission vary across different mucosal sites per individual. They include autoinoculation within one host, direct transmission between individuals (including perinatal transmission and transmission during sexual activity), and indirect transmission through contact with hands. The authors aim to clarify the prevalence and route of transmission per anatomic site, inter-and intra-individually, using a narrative review of the literature.In conclusion, transmission of HPV to the oral cavity and oropharynx is hypothesised to occur mainly through sexual contact. Transmission of particles through saliva has not been proven and daily living activities are not a documented source of HPV infection. Oropharyngeal HPV related cancer survivors and their partners do not show increased risk of infection during sexual intercourse. Transmission of HPV to the oral cavity (autoinoculation with fingers or transmission through saliva in deep kissing) is probably of limited importance. The problem of orogenital transmission concerns very sensitive personal spheres of life and is therefore often unexplored in literature. Furthermore, the literature data on the upper airway clearance mechanisms are still scarce and scattered. Las Infecciones de Transmisión Sexual han adquirido importancia para la salud pública mundial debido a la extrema morbilidad sobre todo en países en desarrollo, por ello fue plantado el objetivo de analizar la prevalencia del virus de papiloma humano y otras infecciones de transmisión sexual. Es un estudio de tipo Descriptivo, Transversal-Retrospectivo, la población estuvo conformada por todas las mujeres diagnosticadas con VPH mediante citología durante el año 2019 en el Hospital General IESS de Manta y por muestreo aleatorio simple se tuvo una muestra total de 71 pacientes. Se obtuvo una prevalencia de infección por VPH en las mujeres atendidas de 21,8%, el tipo de lesión a nivel cervical por presencia de VPH con el mayor número de casos fue de NIC I o de bajo grado con 71,8%, el 77,5% de las mujeres tuvieron una infección previa al virus, siendo Chlamydia trachomatis con 29,60% la patología con mayor relevancia, el 70,4% tuvo su primer acto sexual a los 18 años o menos. Se evidencia la presencia de VPH diagnosticada por citología y de otras infecciones de transmisión sexual en especial de Chlamydia trachomatis en la población femenina estudiada

Frequency of high-risk genotypes of human papilloma virus in oral lesions

Some genotypes of the human papilloma virus (HPV) in the oral cavity cause genetic instability that may lead to cancer. Clinical and histological diagnoses are key tools; however, molecular techniques allow predicting, detecting and monitoring the disease. Objective: To identify the frequency of four high-risk HPV genotypes and their association with lesions in the oral cavity. Materials and Methods: Descriptive cross-sectional study with a sample of 48 patients diagnosed with hyperplastic lesions and others currently classified as potentially malignant disorders (PMDs) of the oral cavity, who underwent biopsies, histopathological analysis, and HPV16, 18, 31, and 45 detection and genotyping by polymerase chain reaction (PCR). Results: Epithelial hyperplasia was the most frequent lesion found in 45.8% (n=22) of patients. Nicotine palatinus and leukoplakia were found in 8.3% and 6.2%, respectively; oral cancer in 6.2%. The total frequency of HPV was 12.5% (6/48). Oral papilloma was found in 6.1% (3/48), and nicotine palatinus and oral cancer in 2.0% each (1/48). HPV16, HPV31, and HPV45 were detected, while HPV18 was not observed. HPV16 was the most frequent genotype found (4 out of 6 patients), while HPV31 and HPV45 were found in one patient each. Only one genotype per lesion was found. The presence of HPV was associated with lesions (χ 2 =11.810; p=0.0375). No significant association with age and gender was found. Conclusion: High-risk HPV continues to be present in oral lesions. The HPV16 viral genotype was the most frequent in the studied lesions.
