Epidemic Cholera in a Crowded Urban Environment, Port-au-Prince, Haiti

Abstract: We conducted a case–control study to investigate factors associated with epidemic cholera. Water treatment and handwashing may have been protective, highlighting the need for personal hygiene for cholera prevention in contaminated urban environments. We also found a diverse diet, a possible proxy for improved nutrition, was protective against cholera.

“…Handwashing with soap and water protects against cholera [16], and may become increasingly important during outbreaks as cholera contamination becomes widespread and disease dissemination is through multiple vehicles [17]. In our study, reporting the presence of soap for handwashing in the home was associated with protection against cholera (aOR 0.41; 95%CI 0.19, 0.87).…”

“…cholerae by in vivo vibriolytic action of the phage and/or nonculturability induced as a host response [26]. Similar to previous studies that have relied on a high positive predictive value of the clinical case definition during cholera outbreaks [4,5,17,27-30], we did not exclude cases on microbiological confirmation of non-cholera aetiologies. The estimates of sensitivity and specificity of the POC test had wide confidence intervals due to a small sample size, however interpretation remained feasible.…”

“…Most patients treated at the CTC live in crowded conditions with limited access to sanitation where drinking water sources included purchased water bags, bottled or filtered water, piped water and water delivered by tanker trucks 12. The GHESKIO camp for IDP established after the earthquake had a population of ∼5,000 during the time of this observation.…”

“…Copper, iron, selenium and zinc are all known to confer function in both innate and adaptive immune systems, with copper also impacting intestinal immune barriers [20]. A history of prior cholera infection has been identified as a risk factor for outbreaks of thiamine deficiency among prisoners [21], and food diversity has recently been shown to be protective against cholera in the ongoing outbreak in Haiti [22]. In our study, it would appear these outbreaks were concurrent and that symptomatic cholera infection did not cause the micronutrient deficiencies among the cases of peripheral neuropathy, as the majority (88%) of cases did not report an episode of diarrhoea in the previous three months.…”
