Background: Interest in the study of combined pathology has recently been explained by the accumulation of new facts, the emergence of new information about interorgan, intertissue and intercellular levels of interactionin the system of the whole organism. Studies of the oral cavity in chronic diffuse liver diseases are of great interest to clinicians, since pathological processes developing in the liver, as a rule, lead to organic and functional disorders in the oral mucosa. The aim of this study was to study the clinical and morphological lesions of the oral mucosa and periodontium in viral hepatitis C. Material and methods: 196 patients were examined, of which: the control group consisted of 100 patients with lesions of the oral mucosa (OM) and periodontal disease who were not diagnosed with HCV and 96 patients of the main study group. Dental status was studied. A pathological examination was carried out. Results: HCV patients group included 96 (63.5% males) and non-HCV group - 100 subjects (62.0% males) with lesions of oral mucous membrane. The lesions of lips and oral mucosa were more frequent in HCV than in the non-HCV group – e.g. erosion (13.5% vs 1%), cracks in the mouth corners (42.7% vs 0%), changes of oral mucosa surface (89.6% vs 3.0%), hemorrhages - (78.1% vs 0%) etc. Conclusion: Thus, during the clinical examination of patients with HCV, the average severity of periodontal damage prevailed. Numerous morphological features can be divided into those that are indicators of the severity and activity of inflammation.

The oral cavity constitutes a unique ecosystem with highly variable ecological niches that harbor a great variety of microorganisms, including yeasts. Molecular methods are currently considered the gold standard for identifying species, although they involve limitations associated with the disruption of yeast cell walls to release the genomic DNA (gDNA) for amplification. Aim: The aim of this study was to compare the performance of different methods for extracting gDNA from Candida albicans and Candida dubliniensis, subsequently amplifying DNA by PCR. Materials and Method: Fifty-two isolates (16 C. albicans and 36 C. dubliniensis) were obtained from subgingival biofilm of HIV+ patients with clinical signs of periodontal disease. The study evaluated 6 gDNA extraction methods and two PCR amplification methods. Furthermore, the presence of alleles of HWP1 gene was determined in C. albicans. Results: Comparisons of six methods show statistically significant differences (p<0.001) except for C. albicans in two of them. For C. dubliniensis, statistical differences were observed in all comparisons. Commercial methods were more efficient for concentrating gDNA than in-house methods, and both PCRs were effective. Ten heterozygous C. albicans isolates for this allele were positive for the HWP1-1 / HWP1-2 allele, one was homozygous for Wild Type HWP1-1 allele, and 5 were homozygous for novel/rare HWP1-2 allele. Conclusions: This study aims to provide simple, inexpensive strategies for phenotypic identification and molecular confirmation of Candida albicans and Candida dubliniensis for non-reference laboratories with low complexity and/or low budgets.

Oral candidiasis is an opportunistic infection of the oral mucosa sustained by fungi of the genus Candida. Various Candida species, with a predominance of C. albicans, normally a saprophyte of the oral cavity, may become virulent and infect the oral mucosa with variegated clinical presentation, in case of imbalance of the oral microbiota, the presence of local predisposing factors and systemic conditions that weaken the immune system. Conventionally, oral candidiasis eradication benefits the use of antifungal drugs. However, the growing phenomena of drug resistance and the increase of infections sustained by non-albicans species are less responsive to common antifungals, thus orienting researches towards the experimentation of alternative therapies. The present review considered the most promising alternative therapeutic proposals. The use of plant derivatives with phytotherapy is a promising option, such as probiotics, to rebalance the oral microbiota in case of dysbiosis. Finally, antimicrobial photodynamic therapy (aPDT), with highly selective fungicidal activity and free of side effects, is also being studied as a powerful alternative to drug administration. All these therapies are alternatives or supportive to the conventional treatment of recurrent and non-drug-responsive forms of oral candidiasis. However, further studies are needed to define the most active compounds, the efficacy of the therapies compared with the conventional ones, and the planning of regulated and standardized protocols.
