Aim To evaluate in vivo combination therapy of systemic parathyroid hormone (PTH) and locally delivered neutral self‐assembling peptide (SAP) hydrogel for periodontal treatment. Materials and Methods Viability/proliferation of rat periodontal ligament cells in a neutral SAP nanofibre hydrogel (SPG‐178) was evaluated using WST‐1 assay. Periodontal defects were created mesially to the maxillary first molars in 40 Wistar rats. Defects were filled with 1.5% SPG‐178 or left unfilled. Animals received PTH (1‒34) or saline injections every 2 days. Microcomputed tomography, histological, and immunohistochemical examinations were used to evaluate healing at 2 or 4 weeks postoperative. Results At 72 hr, cells in 1.5% SPG‐178 showed increased viability/proliferation compared to cells in 0.8% SPG‐178 or untreated controls. In vivo, systemic PTH resulted in significantly greater bone volume in the Unfilled group at 2 weeks (p = .01) and 4 weeks (p < .0001) than in the saline control. At 4 weeks, a significantly greater bone volume was observed in the PTH/SPG‐178 (p = .0003) and PTH/Unfilled (p = .004) groups than in Saline/SPG‐178 group. Histologically, greater bone formation was observed in PTH/SPG‐178 at 4 weeks than in other groups. In the PTH/SPG‐178 group, increased proportions of PCNA‐, VEGF‐, and Osterix‐positive cells were observed in the treated sites. Conclusions These findings suggest that intermittent systemic PTH and locally delivered neutral SAP hydrogel enhance periodontal healing.

Abstract:Here we report the preparation and characterization of a novel biomimetic toothpaste containing morphogenetically active amorphous polyphosphate (polyP) microparticles enriched with retinyl acetate ("a-polyP/RA-MP"). The spherical microparticles (average size, 550±120 nm), prepared by co-precipitating sodium-polyP with calcium chloride and supplemented with retinyl acetate, were incorporated into a basis toothpaste at a final concentration of 1% or 10%. The paste containing "a-polyP/RA-MP" significantly increased the growth of human mesenchymal stem cells (MSC), compared to a commercial toothpaste which acts rather inhibitory and the paste without polyP and retinyl acetate. qRT-PCR experiments revealed that the retinoid causes an induction of the expression of the MSC marker genes for osteoblast differentiation encoding collagen type I and alkaline phosphatase. On the other hand, the polyP ingredient, supplied as Zn-polyP microparticles ("Zn-a-polyP-MP") strongly inhibited the growth of the cariogenic bacterium Streptococcus mutans.We demonstrate that the amorphous polyP-containing toothpaste, enriched with retinyl acetate, efficiently repairs both cracks/fissures and carious lesions in the tooth enamel, and reseals dentinal tubules, already after a 5 d treatment (brushing) of teeth twice daily for 5 min as examined by SEM and quantitative EDX analysis. The stability of the occlusion of dentin cracks even turned out to resist against short high power sonication treatment. Our results demonstrate that the novel toothpaste prepared here, containing amorphous polyP and retinyl acetate, is particularly suitable for prevention/repair of (cariogenic) damages of tooth enamel/dentin and for treatment of dental hypersensitivity.

Background This study aimed to investigate the regenerative effects of 2,3,5,4′‐tetrahydroxystilbene‐2‐O‐β‐d‐glucoside (THSG)‐treated human dental pulp stem cells (DPSC) on the healing of experimental periodontal defects in rats. Methods The maxillary first molars of 30 male Sprague–Dawley rats were extracted, and after healing, bilateral periodontal defects were surgically created mesially in second molars. The defects were treated with Matrigel (as control), DPSC, or DPSC + THSG. After 2 weeks, the healed defects were evaluated using microcomputed tomography and through histological and immunohistochemical analyses. Results In the microcomputed tomography analysis, more new bone formation in the DPSC and DPSC + THSG groups was observed compared with the control group. The periodontal bone supporting ratio in site with DPSC + THSG was significantly higher than that in DPSC. Histologically, an enhanced new bone formation and more significant periodontal attachment were observed in the DPSC + THSG group. The expression levels of proliferating cell nuclear antigen (PCNA), vascular endothelial growth factor (VEGF), and osteopontin (OPN) in the DPSC + THSG group were significantly greater than those in other groups. Conclusions THSG‐revolutionized DPSCs significantly shortened the regenerative period of periodontal defects by enhancing the cell recruitment and possibly the angiogenesis in rat models, which illustrate the critical implications for a clinical application and provide a novel tactic for periodontitis treatment.
