lthough endovascular repair of thoracic aortic aneurysms is now an attractive alternative to surgical graft replacement, 1-6 many patients are excluded because of unfavorable anatomy, particularly an inadequate proximal landing zone. The outcome of endovascular stent grafting for thoracic aortic disease is largely dependent on the anatomical factors of the aneurysm or dissection, 7,8 not only the presence of suitable landing zones, but also because the dilated landing aorta is much wider than 38 mm in diameter, which is beyond the upper limit for most current commercial devices. We describe how we successfully extended the application of endovascular stent grafting to treat arch aneurysms using less invasive hybrid surgery without extracorporeal support. These procedures are combined with banding the proximal landing aorta after debranching of the aortic arch and relocation of the supra-aortic branches. MethodsSince October 2005, 196 patients have undergone endovascular repair of aortic aneurysm (108 abdominal and 88 thoracic aortic aneurysms). One to four per cent of all deaths in patients over 65 are caused by aneurysmatic diseases of the abdominal or thoracic aorta. For elective surgery in abdominal aneurysms, open surgery and endovascular treatment both demonstrate brilliant overall results. In the thoracic aorta, new endovascular procedures have led to considerable reductions of postoperative morbidity and mortality. Nevertheless, in view of the endovascular procedure's high cost and the still unclear long-term behaviour of the stent device, a second opinion from a specialised centre is an absolute necessity.

Endovascular Abdominal Aortic Aneurysmal Repair: A Korean Perspective

Endovascular aneurysm repair (EVAR) was initially introduced as a less invasive alternative to conventional open repair. EVAR was subsequently adopted as a treatment option for abdominal aortic aneurysm. In Korea, open repair is more widely available than EVAR, although EVAR can be performed in several hospitals. Due to the rapidly aging population in Korea, there has been a shift from private healthcare to government-regulated universal coverage and EVAR may be a more feasible option for cardiovascular interventionalists in these days. The improvement of EVAR was rapidly attained by many pioneers for the last two decades. Although issues such as indications and durability of EVAR remain to be elucidated, its application can be extended further of milder invasiveness-related effects on comorbidities and less discomfort to patients. Aortic stentgrafting has been performed for various aortoiliac pathologies over the last 13 years at our cardiovascular center.
