Background. The topicality of the theme is due to the significant prevalence of cardiovascular diseases and an increase in costs for medical care, the high risk of disability, which characterizes the medical and social component of the problem. Pathological changes can be a manifestation not only of a cardiovascular disorder, but also develop as a result of other diseases, including diabetes mellitus, obesity, and thyroid pathology. One of the main mechanisms of morbidity and mortality is macrovascular complications that can occur in endothelial dysfunction and oxidative stress. The purpose of the research is to study the peculiarities of changes in the parameters of the endothelial system and oxidant status in animals with insulin resistance and obesity under conditions of adequate iodine supply and iodine deficiency. Materials and methods. Study included 75 sexually mature rats having received a high-carbohydrate, high-fat diet under conditions of adequate and limited iodine supply, followed by analysis of markers of carbohydrate metabolism, thyroid status, indices of endothelial function, lipid peroxidation and antioxidant protection. Results. The development of insulin resistance and obesity in a diet loaded with fructose and fats is accompanied by the development of endothelial dysfunction: in the blood serum, the level of endothelin-1 increases and inducible NO-synthase (iNO-synthase) is activated, in the myocardium, the activity of iNO-synthase increases compared to the data in animals who received a standard diet. The development of oxidative stress in experimental animals characterizes an increase in the content of diene conjugates and thiobarbituric acid-reactive substances in blood serum and myocardium against the background of inhibition of serum antioxidant enzymes (catalase, superoxide dismutase, ceruloplasmin, glutathione peroxidase, glutathione reductase). The degree of endothelial dysfunction and the intensity of lipoperoxidation increase with hypothyroid dysfunction against the background of iodine deficiency. Conclusions. Metabolic disorders under the conditions of insulin resistance and obesity are characterized by the development of endothelial dysfunction and oxidative stress, which are the predictors of the development of cardiovascular risks. Their intensity depends on carbohydrate and thyroid homeostasis.

РЕЗЮМЕ. Хронічна етанолова інтоксикація супроводжується пошкодженням майже всіх органів та систем організму. Особливості ремоделювання артеріального русла передміхурової залози при даній патології вивчені недостатньо. Мета – морфометрично дослідити структурні зміни артерій передміхурової залози в умовах хронічної алкогольної інтоксикації. Матеріал і методи. Досліджена передміхурова залоза 62 лабораторних статевозрілих білих щурів-самців, які були поділені на 2 групи. Перша група (30 тварин) служила контролем. Щурам другої групи (32 тварини) внутрішньошлунково вводили 30 % розчин етанолу з розрахунку 2 мл на 100 г маси тіла протягом 28 діб один раз на добу. Евтаназію щурів здійснювали кровопусканням в умовах тіопенталового наркозу через 30 днів від початку досліду. На мікропрепаратах передміхурової залози досліджували артерії середнього та дрібного калібрів, де визначали їх зовнішній та внутрішній діаметри, товщину медії, адвентиції, індекси Вогенворта та Керногана, висоту ендотеліоцитів, діаметр їх ядер, ядерно-цитоплазматичні відношення у цих клітинах, відносний об’єм ушкоджених ендотеліоцитів. Кількісні величини оброблялися статистично. Результати. Встановлено виражене ремоделювання артерій передміхурової залози в умовах хронічної алкогольної інтоксикації. Виявлено потовщення стінки артерій, зростання товщини медії, адвентиції, звуження їх просвіту, зміни індексів Вогенворта та Керногана, пошкодження ендотеліоцитів, порушення в них ядерно-цитоплазматичних відношень. Структурні зміни домінували в артеріях дрібного калібру передміхурової залози. Висновки. Проведені дослідження та отримані результати свідчать, що хронічна алкогольна інтоксикація у лабораторних статевозрілих білих щурів-самців призводить до вираженого ремоделювання артеріального русла передміхурової залози, яке характеризується потовщенням стінки артерій, звуженням їх просвіту, істотними змінами індексів Вогенворта і Керногана, атрофічними, дистрофічними та некробіотичними змінами ендотеліоцитів, порушенням у них структурного клітинного гомеостазу, ендотеліальною дисфункцією, гіпоксією, дистрофічно-некротичними змінами клітин та стромальних структур, інфільтрацією та склерозуванням. Ступінь структурних змін домінує у артеріях дрібного калібру.

Reduction of the risk of failure to achieve blood pressure control in patients with arterial hypertension and coronary heart disease: the role of quercetin

Aim. Determine the effect of quercetin on predictors of not reaching target blood pressure (BP) levels in patients with arterial hypertension (AH) and coronary heart disease (CHD). Materials and methods. 120 patients with stable coronary heart disease and hypertension of the II stage 2–3 degrees (66 women and 54 men) were examined. The patients were divided into 2 groups: Group I – 58 patients who, in addition to basic therapy (ramipril/amlodipine), received quercetin (Corvitin®), Group II – 62 patients who received only basic antihypertensive therapy. The parameters of daily blood pressure monitoring, blood lipid spectrum, C-reactive protein, the concentration of adhesion molecules (s-VCAM, s-ICAM-1), levels of endothelin-1 (ET-1), IL-1, IL-6, TNF-α were determined. Statistical processing of materials was carried out using the following programs: Minitab 21.0, StatPlus Pro 7.6.5, Wizard Pro, Jamovi, MedCalc. Results. We have established that the risk of not reaching the target level of daily SBP in the group of basic treatment of hypertension (group II) significantly increases with exceeding the level of LDL >4.0 mmol/l (by 3.6 times), an increase in the level of IL-6 >7 ng/ml (by 10 times), an increase in the level of TNF-α >9 pg/ml (by 7 times), an increase in the level of VCAM >1100 ng/ml (by 2.4 times). In the group of people who additionally took quercetin (group I), with an increase in the level of ET-1 >1.1 pg/ml, the risk of failure to reach the target SBP increases by 2.6 times, in the II group – by 3.5 times toward to daily SBP and 8.6 times – toward to daily DBP. We found out that an increase in the concentration of IL-6 > 7 ng/mg significantly increases the risk of arrhythmias by 6 times in group II. Conclusions. When quercetin is used, it is possible to achieve the target values of SBP (daily, daytime, nighttime) and DBP (daily, daytime, nighttime) in a significantly higher proportion of patients compared with those who are prescribed only standard antihypertensive therapy. Quercetin has an endothelial protective effect, as evidenced by a significant decrease in the levels of vascular endothelial adhesion molecules (s-VCAM), type I intercellular adhesion molecules (s-ICAM-1), ET-1, IL-1, IL-6, TNF-α. The additional use of quercetin against the background of standard antihypertensive therapy significantly reduces the risk of failure to achieve the target levels of SBP, DBP and the risk of heart rhythm disturbances even against the background of increased levels of proinflammatory cytokines and markers of endothelial dysfunction.
