Asymmetric dimethylarginine (ADMA) has evolved as an important regulator of nitric oxide (NO) synthesis in recent years. Elevated levels of ADMA have been reported in many conditions associated with a high cardiovascular risk. Moreover, ADMA is a biomarker for major cardiovascular events and mortality in cohorts with high, intermediate and low overall cardiovascular risk. Discrepant data have been reported on cardiovascular risk in people with and without diabetes mellitus, and the association of ADMA with diabetes mellitus per se has also remained controversial, possibly relating to type and stage of diabetes. Clinical and experimental data suggest that there is a multifaceted link between ADMA and insulin metabolism and action on one hand, and ADMA and glucose utilisation on the other. This interplay may be regulated by the enzyme involved in the metabolic degradation of ADMA, dimethylarginine dimethylaminohydrolase (DDAH). Recent data from prospective clinical studies suggest that whilst ADMA may be a marker for total mortality in patients without diabetes, elevated ADMA may exert beneficial effects in patients with diabetes. In this respect, ADMA could serve as a re-coupling agent overcoming endothelial nitric oxide synthase (eNOS) uncoupling in patients with diabetes. Anticipated advances in clinical and experimental investigation will help us to better understand this complex interrelationship between diabetes, eNOS, DDAH and ADMA.

Ö ÖZ ZE ET T A Am ma aç ç: : Çocukluk çağında primer hipertansiyonun yüksek kardiyovasküler mortalite ve morbidite ile ilişkisi son yıllarda ciddi bir sağlık sorunu hâline gelmiş ve giderek önem kazanmıştır. Bu çalışmada, primer hipertansiyonlu çocuklarda geleneksel ve geleneksel olmayan risk faktörleri ve bu faktörlerin hedef organ hasarı üzerine etkisinin araştırılması amaçlanmıştır. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Primer hipertansiyon tanısı alan 7-18 yaş arasındaki 50 hasta ile aynı yaş grubunda sağlıklı 100 kişi kontrol grubu olarak çalışmaya alındı. Hasta grubuna 24 saatlik yaşam içi kan basıncı ölçümü yapıldı. Biyokimyasal olarak kan lipitleri, hs CRP, Lp(a) bakıldı. Hedef organ tutulumu açısından retinopati, nefropati (24 saatlik idrarda mikroalbuminüri), kardiyopati (sol ventrikül hipertrofisi ve sol ventrikül kitle indeks) ve vaskülopati (karotis intima mediya kalın-lığı ve arteriyel katılık) değerlendirildi. B Bu ul lg gu ul la ar r: : Toplam 50 (35 erkek, 15 kız) hastada ortalama yaş 13,1±3,2 yıl, 100 (54 erkek, 46 kız) sağlıklı kontrolde 13,06±2,7 yıl idi. Hastaların 17 (%34)'sinde göz dibi etkilenmesi mevcuttu (14 (%28)'ünde evre 1, 3 (%6)'ünde evre 2 hipertansif retinopati). Bir (%2) hastada ekokardiyografik olarak sol ventrikül hipertrofisi görüldü. Sol ventrikül kitle indeksi 7 (%14) hastada yüksek saptandı (Ort: 28,32±11,43 g/m2,7). 26 (%52) hastada cIMT (sağda 0,468±0,062, solda 0,472±0,064 mm, kontrol grubunda sağda 0,406±0,041, solda 0,405±0,041; p=0,000), 18 (%36) hastada PWV (5,76±0,84 m/s, kontrol grubunda 5,30±0,70 m/s; p=0,000), 11 (%22) hastada santral augmentasyon indeksi (%10,54±6,56, kontrol grubunda %9,41±8,54; p=0,366) yüksek bulundu. Hastalığın şiddeti arttıkça mikroalbuminüri ve inflamasyon göstergelerinin anlamlı olarak arttığı, HDL değerlerinin düştüğü görüldü. Beyaz önlük hipertansiyonu saptanan 8 hastada değişik derecelerde hedef organ tutulumu saptandı.
