In “What should be known prior to performing EUS exams, Part I,” the authors discussed the need for clinical information and whether other imaging modalities are required before embarking EUS examinations. Herewith, we present part II which addresses some (technical) controversies how EUS is performed and discuss from different points of view providing the relevant evidence as available. (1) Does equipment design influence the complication rate? (2) Should we have a standardized screen orientation? (3) Radial EUS versus longitudinal (linear) EUS. (4) Should we search for incidental findings using EUS?

Transoesophageal ultrasound imaging of the spinal canal through an intervertebral disc was successfully achieved in 44 of 50 adults (18 women and 32 men; mean age 56 years, range 22-79) who, for cardiological indications had transoesophageal echocardiography. On average, five (range 1-11), usually sequential, segments were visualized. In 37 patients pulse-synchronous sagittal displacement of the spinal cord was noted, presumably caused by cerebrospinal fluid pulsation. In one woman, with known syringomyelia, the spinal cord cavity was well shown. Real-time demonstration of dynamic events in the spinal canal opens up new possibilities for the diagnosis and follow-up monitoring of spinal diseases.

Gastroscopic ultrasound tomography enables a direct sonographic approach to the upper gastrointestinal tract and the surrounding organs. First experiences show that this new method is characterised by a great diagnostic potential. Its advantages compared with conventional sonography are the avoidance of sonographic obstacles and an improved recognisability of details; part of the examined area is, so to say, enlarged due to the use of high ultrasonic frequencies. With an ultrasonic frequency of 7.5 megahertz, the following processes can be visualised more clearly than with conventional sonographic equipment: processes in the pancreas, cholelithiasis of the common bile duct, hepatic foci, processes in the splenorenal angle, as well as tumours of the stomach and of the esophagus. In cardiology, clear indications are easily recognised, such as the atrial septal defect.
