-Introduction: Surgical techniques for wisdom tooth extraction are very well described and codified in the literature. As far as standard approaches are concerned, oral surgeons enjoy a wide array of therapeutics and methods to remove impacted third molars. However, conventional intra-oral first path is not suitable when it comes to atypical inclusion and/or limited access. Observation: This paper illustrates extreme situations where the intraoral armamentarium shows its weaknesses: first, two cases where modified submandibular access was successfully used to remove deeply impacted lower third molars; and 1 case where Lefort I osteotomy was achieved to remove a maxillary third molar projected into the pterygo-maxillary fossa. Discussion: The extra-oral surgical approach for dental extraction is very rare and in most cases the classical intra-oral approach dominates. However, when intraoral surgery of the pterygo-maxillary fossa is impossible by the conventional endobuccal method, appropriate extra-oral approaches might be useful. Conclusion: Rarely, atypical surgical approaches may be used for removal of ectopic third molars.Résumé -Approches chirurgicales atypiques pour l'avulsion de troisiemes molaires ectopiques : a propos de 3 cas. Introduction : Les techniques d'avulsion de dents de sagesse sont un sujet très bien documenté dans la littérature. Tant que leur position est classique, les chirurgiens ont à disposition un large éventail de thérapeutiques et de méthodes afin d'avulser ces dents. Dès qu'elles ont une position atypique ou un accès chirurgical très limité, il n'y a plus de consensus. Observation : Cet article met en avant des situations où l'abord intra-oral classique trouve ses limites : deux cas cliniques illustrent une voie d'abord sub-mandibulaire modifiée pour l'avulsion de dents de sagesse mandibulaires en position très basse, et un troisième cas clinique décrit une voie d'abord de type Lefort I pour l'avulsion d'une dent de sagesse maxillaire ayant migré dans la fosse ptérygo-maxillaire. Discussion : L'approche chirurgicale extra-orale pour une avulsion dentaire est exceptionnelle et dans la plupart des cas, une approche conventionnelle intra-orale suffit. Mais, lorsque une approche chirurgicale intraorale de la fosse ptérygo-maxillaire est impossible, une voie d'abord extra-orale appropriée peut être utile. Conclusion : Rarement, une approche chirurgicale atypique pour les avulsions de dents de sagesse ectopiques peut être utile.

Résumé -Introduction :La dentition est un phénomène complexe amenant les dents en position fonctionnelle sur l'arcade. L'éruption dentaire est émaillée d'aléas. Une dent ectopique est une dent dont l'éruption se fait dans un site plus ou moins éloigné de la normale. Les dents ectopiques peuvent être responsables de manifestations cliniques dont le traitement passe par l'avulsion qui présente souvent de réelles difficultés. L'objectif principal de cet article est de présenter les arguments ayant conduit à choisir une voie d'abord atypique par ostéotomie de Le Fort I (OLFI) pour réaliser l'exérèse d'une troisième molaire maxillaire ectopique intrasinusienne et de son kyste. Observation : Il s'agit d'un patient de 26 ans consultant pour une sinusite droite unilatérale et un écoulement endobuccal postérieur récurrent. Dans ses antécédents, on note l'avulsion de trois dents de sagesse avec notion d'agénésie de la 18. Le panoramique dentaire montre une image radio-opaque dans le sinus maxillaire droit. Le scanner a confirmé le diagnostic de troisième molaire intrasinusienne avec une image hyperdense à sa partie postéro-supérieure, et autour une volumineuse image d'allure kystique entourée d'une cloison osseuse. Nous avons réalisé une voie d'abord type OLFI. Les suites opératoires ont été simples. À six mois, le patient est asymptomatique. Discussion : Le principal problème posé par ce cas est le choix de la voie d'abord. La voie de Caldwell-Luc aurait conduit à un délabrement important de la paroi antérieure du sinus. L'OLFI permet la résection de la cloison osseuse, l'exérèse de la lésion en monobloc et le curetage du sinus sous contrôle visuel. L'OLFI est donc la voie d'abord la plus adaptée.Abstract -An ectopic maxillary intra-sinus third molar: Le Fort I osteotomy approach. Introduction: Teething is a complex phenomenon leading to the functional position of teeth within the dental arch. Numerous hazards are associated with dental eruption, such as an ectopic tooth. An ectopic tooth occurs when the tooth eruption is adjacent to the normal site. The main problem with ectopic teeth lies in their extraction. This article presents arguments which justify the selection of an atypical route to extract an ectopic maxillary intra-sinus wisdom tooth and cyst, through a Le Fort I osteotomy (LFIO). Clinical observation: A 26-year-old patient presented recurring signs of right unilateral sinusitis and an intraoral posterior flow. The patient's history noted the extraction of three wisdom teeth with agenesis of the upper right one. The orthopantomogram showed a radiopaque image in the right maxillary sinus. The scan imagery confirmed the diagnosis of an intra-sinus wisdom tooth with a hyper-dense image in its upper posterior part. The scan also showed a hyper-dense image of cystic appearance around the tooth, surrounded by a bony septum. An LFIO procedure was performed. The post-operative course was uneventful.
