Endoscopic radiofrequency ablation for malignant biliary obstruction: a nationwide retrospective study of 84 consecutive applications

Abstract: Except for one severe interventional complication (hepatic infarct), RFA presented as a technically feasible and safe therapeutic option for the palliative treatment of malignant biliary obstruction. The good results of stent patency and survival in this study should be proven in prospective (controlled) trials to further quantify the efficacy of this promising new technique.

“…Radiofrequency ablation seems to prolong survival in inoperable iCCA 256 . Intraductal radiofrequency ablation has been shown to be safe and feasible in the treatment of extrahepatic CCA 257 and seems to be a safe way of improving stent patency 258 . On the other hand, photodynamic therapy can improve survival in patients with unresectable CCA [259][260][261] .…”

“…Only three studies have reported a similar occurrence following intrabiliary RFA, including six cases of hemobilia and one case of hepatic artery pseudoaneurysm rupture (14,16,17). In the present case, endoscopy was performed to identify the bleeding site, but no injury or active bleeding was observed in the duodenum.…”

“…There have been reports of the use of intraductal RFA for the treatment of unresectable malignant biliary obstructions with percutaneous and endoscopic approaches, demonstrating its feasibility and safety; pain, cholangitis and asymptomatic biochemical pancreatitis have been reported as the only common complications (2,6,13), while hepatic coma, newly diagnosed left bundle branch block and partial liver infarction have been reported as uncommon complications (2,14,15). However, a few cases of hemobilia and mortality following intrabiliary RFA have also been reported (15).…”

“…One case of partial liver infarction was reported in a patient with a Bismuth IV stage Klatskin tumor after RFA procedure. 51 No other serious complications have been reported so far. According to our knowledge, the endobiliary RFA technique has not been tested so far for clearance of occluded SEMS in malignant biliary obstruction.…”
