Endoscopic Evaluation of the Quality of the Anastomosis After Esophagectomy with Gastric Tube Reconstruction

Abstract: Cervical anastomosis resulted in a better outcome for esophagogastrostomy by lowering the risk of reflux esophagitis; this outcome might improve the patient's quality of life.

“…Thus, we pulled up the stomach to encase the anastomosis by a stapler after the anastomosis procedure. This measure increases the pressure that the cervical muscles and soft tissue place on the anastomotic site, enhances the pressure difference between the oesophagus and the stomach at the anastomosis and helps relieve postoperative gastro-oesophageal reflux [19][20][21][22].…”

“…A seleção cuidadosa de pacientes para a operação, visando a redução da morbidade pós-operatória, é atitude enfatizada e preconizada pela literatura médica 7,32,33,54,62 29,30,56 . Valorizam-se os níveis séricos de albumina 17 , sendo excepcional pelos autores a realização da operação em vigência de valores inferiores a 3g/dL.…”

“…The physiologic explanation for this is that with a lower anastomosis , more of the stomach is subject to positive intra-abdominal pressure, which pro motes greater reflu x [27]. In addit ion, the results of Kim's study [28] showed that the incidence of reflu x esophagitis was significantly lo wer in the cervical anastomosis group. Cerv ical anastomosis have shorter pro ximal esophageal segments compared to the longe r intrathoracic segments, which are subject to weakened propulsion.…”
