We assessed the association of surgically verified endometriosis and risk of non-gynecological cancers according to the type of endometriosis (i.e., ovarian, peritoneal and deep infiltrating endometriosis). All diagnoses of endometriosis combined with relevant procedural codes were identified from the Finnish Hospital Discharge Register 1987-2012. Non-gynecological cancers diagnosed after the endometriosis diagnosis were obtained from the Finnish Cancer Registry. The cohort of 49,933 women with surgically verified endometriosis and the sub-cohorts of ovarian (n = 23,210), peritoneal (n = 20,187), and deep infiltrating (n = 2,372) endometriosis were analyzed separately. The endometriosis cohort contributed 838,685 person-years of follow-up and the Finnish female population served as the reference cohort. The standardized incidence ratio (SIR) and 95% confidence interval (95%CI) was calculated for each cancer separately. The follow-up ended at emigration, death or on the 31st of December 2014. The non-gynecological cancer risk was not increased among women with endometriosis (SIR 1.03, 95%CI 0.98-1.08). Endometriosis was associated with an increased risk of thyroid cancer in the entire cohort (SIR 1.43, 95%CI 1.23-1.64) and in the sub-cohorts of ovarian and peritoneal endometriosis. We found a decreased risk of mouth and pharynx cancer (SIR 0.60, 95%CI 0.41-0.80), and of pancreatic cancer (SIR 0.76, 95%CI 0.58-0.96). The incidence of basal cell carcinoma was elevated in the entire cohort (SIR 1.18, 95%CI 1.10-1.25) and in the sub-cohorts of ovarian and peritoneal endometriosis. In conclusion, women with surgically verified endometriosis have an altered risk of only few non-gynecological cancers.

Hypothesis/aims of study. The current analysis was undertaken to assess the incidence of malignant diseases in patients with endometriosis. Study design, materials and methods. This is a cohort study of women with surgically verified endometriosis retrieved from the Saint Petersburg 122 Clinical Hospital Discharge Register 1996–2006 (n = 1551). Data were linked to the Saint Petersburg Cancer Agency to identify cases of malignancy. Results. Malignant diseases were identified in 6.3% (n = 98) of cases. The median follow-up was 12.2 (7.5) years. Breast cancer (n = 29), ovarian cancer (n = 7), endometrial cancer (n = 7), thyroid cancer (n = 7), colorectal cancer (n = 6), melanoma (n = 6), non-Hodgkin’s lymphoma (n = 4), and glioblastoma (n = 3) were prevalent. Conclusion. The incidence of cancer in patients with endometriosis is low. Women with endometriosis have an increased risk of some malignancies, particularly ovarian cancer.

Introduction: Endometriosis is a common gynaecological disorder associated with pelvic pain and sub-fertility, affecting 7-15% of women of reproductive age. The disease is defined as the presence of endometrial-like tissue outside the uterine cavity, primarily on pelvic organs. The aetiology of endometriosis is still uncertain. The disease rarely causes mortality, however, it may have a significant impact on a patient's quality of life. Moreover, several studies have consistently shown that endometriosis is associated with a higher risk of some types of malignancies. Results: Endometriosis shares several molecular characteristics with invasive cancer, such as inflammation, tissue invasion, angiogenesis, dysfunction of immune cells and pro-survival features. Studies have shown that ovarian cancer, especially clear cell, endometrioid and serous, has been associated with endometriosis. Furthermore, the risk of cancer of the uterine cervix has shown to be decreased and the results on the risk of the cancers of uterine corpus or breast are inconsistent. Among non-gynecological cancers, melanoma of the skin, non-Hodgkin-lymphoma, brain and thyroid cancers have also been associated with endometriosis. Conclusions: Research results indicate that women with endometriosis have an increased risk for some types of malignancies, therefore, they should be under strict gynecological control, even many years after menopause. Further research is needed to prove the real risk of different types of malignant tumors among women with endometriosis. In addition, it is necessary to establish periodic examination schedules among these patients in order to prevent certain types of malignancies.
