The incidence and severity of endometriosis in adolescent are comparable with the incidence in adult women. The mean delay between the onset of symptoms and the final diagnosis varies between 6.4 and 11.7 years. The longer the diagnosis is delayed, the more the endometriosis can progress to a more severe stage certainly in the group of patients with pelvic pain. The evolution of endometriosis and its progressivity are not predictable, and the severity of the disease is not directly related to the degree of pain. Endometriotic cysts have a detrimental effect on the ovarian reserve by the evolution in time and the surgical excision technique. Already, in small endometriotic cysts (<4 cm), loss of follicular reserve is present together with the formation of fibrosis in the cortex of the ovary. Early diagnosis of endometriosis in the adolescent deserves our full attention. Non-invasive imaging techniques like 2-D and 3-D ultrasound are helpful in the early diagnosis. Early ablative surgery is recommendable. Although laparoscopy is traditionally recommended, transvaginal laparoscopy has been shown to be most effective in ablating endometriomas with a maximum diameter of 3 cm. Early detection and intervention will contribute to a better quality of life in these adolescents and also to a lower damage of the ovarian tissue by a less invasive ablative surgery.

about the "progressive nature" of early-onset disease was not shared by Evers who argued in an Editorial that in the seven published trials where laparoscopy was performed before and after a period without treatment, in 71% of the cases the disease did not progress (2). Two considerations are in order when analyzing Evers' conclusion. On the one hand, it is clearly impossible to predict in which cases endometriosis will progress; on the other, the investigations upon which Evers based his conclusions were conducted in adult women and, over the last few years, we have argued that there is strong indirect evidence that the adult and the pre-pubertal and adolescent variants of endometriosis may have a different origin and phenotype (3). According to this hypothesis, early-onset disease traces its roots in the retrograde fetal endometrial shedding occurring in approximately 5% of neonates at the time of birth that is manifested by a well-documented, but neglected phenomenon called "neonatal menstruation" (4, 5). The visible bleeding with retrograde endometrial shedding has the ability to affect pelvic organs and become the source of disorders that may develop later in life and pose a risk for the future reproductive health of the adolescent (3, 6-8). sis was first described in teenagers as early as the 1940s (4), early studies suggested that endometriosis was rare during adolescence (5). Since endometriosis can only be diagnosed at laparoscopy, the true prevalence of endometriosis among adolescents remains unknown. Furthermore, there has been limited research on endometriosis in adolescents. The aim of this paper is to present a systematic review of characteristics and management of endometriosis in adolescents. Materials and methods The literature review was done using the electronic database PubMed focusing on the terms 'adolescents', 'endometriosis', 'teenagers', 'pain', 'infertility', 'quality of life', 'medical' and 'surgical management'. The search of literature was set from 1980 onward because the international classification of endometriosis was revised in 1979 by the revised American Fertility Society (rAFS). It was limited to articles in English language that addressed the research question at hand. Articles were only included if they reported original research. The reference lists of all selected articles were reviewed to identify additional papers.
