Endometrioma ≤3 cm in Diameter per se Does Not Affect Ovarian Reserve in Intracytoplasmic Sperm Injection Cycles

Abstract: Background: Our aim was to determine the effect of single endometriomas ≤3 cm in diameter per se on ovarian reserve in intracytoplasmic sperm injection (ICSI) cycles. Methods: We enrolled 19 consecutive infertile patients (29 cycles) who had unilateral single endometriomas ≤3 cm in diameter and who underwent ICSI. Results: The mean age of the patients was 33.3 ± 4.9 years. The mean diameter of endometriomas was 21.8 ± 4.9 mm. Left- and right-sided endometriomas were 34.5 and 65.5%, respectively. The number of … Show more

“…In this context, size of the endometrioma seems to be the crucial determinant in management. Esinler et al demonstrated that the unilateral endometriomas ≤3 cm in diameter did not have a deleterious effect on ovarian reserve in ICSI cycles (29). Recently, Cocia et al observed a critical endometrioma size of 3 cm, in those with unilateral cyst, above this size, which the total numbers of follicles and retrieved oocytes were negatively affected (26).…”

“…Pooling of results from four studies Ashrafi et al, 2014;Esinler et al, 2012;Filippi et al, 2014) that reported the number of oocytes retrieved between affected ovary and intact ovary ( Figure 5B) did not show a significant difference (WMD −0.11; 95% CI −0.25 to 0.03)]. The I 2 value was 0%, indicating that heterogeneity was unlikely between the studies.…”

“…A few studies have shown that women with endometriosis have decreased ovarian reserve, decreased AFC and decreased ovarian response to gonadotropin [8,9,10]. In contrast, one study done on patients undergoing intracytoplasmic sperm injection has shown no deleterious effect on ovarian reserve in women who have milder forms of endometriosis, specifically small endometriomas [11]. Accordingly, left lateralization of endometriosis might be associated with decreased number of antral follicles in the left ovary.…”
