Objective: To compare natural vs. hormone replacement treatment (HRT) for cryo-thaw embryo transfer cycles in patients with frozen embryos from previous ART. Design and Settings: Retrospective chart review of 164 patients (242 cycles) who underwent natural or HRT cryo-thaw embryo transfer between January 2002 and April 2005. Main Outcome Measures: Pregnancy rates per transfer in natural and HRT cryo-thaw cycles. Results: The pregnancy rate per transfer was higher with natural cycles (36.76% vs. 22.99%, P = 0.0298). There was no statistical difference in mean age, endometrial thickness, and average embryo quality in successful cycles. Mean endometrial thickness prior to transfer was greater in natural cycles (9.95 vs. 8.89 mm, P < 0.001). Mean serum estradiol levels were higher in the HRT group prior to transfer (526.1 vs. 103.8 pg/ml, P < 0.001), and were found to be lower in women who achieved pregnancy (337.1 vs. 433.3 pg/ml, P = 0.0136). Conclusion: Hormone replacement in preparation for cryo-thaw transfer of embryos was found to be associated with decreased pregnancy rates in comparison to natural cycle cryo-thaw transfer. Greater endometrial thickness was achieved with lower serum estradiol levels in patients undergoing natural cycles, suggesting Financial disclosure: none of the above authors had any financial interests with commercial companies.

Amaç: Bu çalışmada 39 yaş ve üzeri infertil kadın hastalarda yaşın, prognostik faktörler ile üreme potansiyeli üzerindeki sonuçlarının değerlendirilmesi amaçlandı. Gereç ve Yöntemler: İleri yaşta (≥39 yıl) 107 infertil kadın hasta retrospektif olarak değerlendirildi. Hastaların insan koryonik gonadotropin (hCG) uygulanan günde ölçülen endometrium kalınlığı, toplam kullanılan gonadotropin dozu, stimülasyon süreleri, 3. gün bazal follikül stimüle edici hormon (FSH) ile bazal estradiol (E 2 ) düzeyleri, toplanan oosit ile fertilize oosit sayıları ve transfer edilen embriyo sayıları kaydedildi. Bu parametrelerin gebelik başarısı üzerine etkileri değerlendirildi. Hastalar 39 yaş (n=47), 40-41 yaş (n=37) ve 42 ve üzeri yaş (n=23) olmak üzere üç yaş grubu karşılaştırıldı. Bulgular: Canlı doğum oranı 39 yaş grubunda %23,2 ve 40-41 yaş grubunda %14,7 olarak gözlendi. Kırk iki ve üzeri yaş grubunda ise canlı doğum görülmedi. Bu çalışmanın sonuçları yaşın, toplanan oosit ve fertilize oosit sayısının gebelik gelişimi üzerine etkisinin olduğunu gösterdi. Sonuç: Gebelik oranı 39 yaşından sonra hızlı bir şekilde düşmektedir; intrasitoplazmik sperm enjeksiyonu sonrası, hastalarda 39 yaşından itibaren gebelik başarısı 3 yıl sonra 9 kat azalmıştır. The endometrial thickness on the day of human chorionic gonadotropin (hCG), total dose of gonadotropin used, duration of stimulation, levels of day-3 basal follicule stimulating hormone (FSH) and basal estradiol (E 2 ), number of retrieved oocytes and fertilized oocytes, and number of embryos transferred were recorded. These parameters were used to evaluate their effects on the success of pregnancy. Patients were categorized into three age groups: 39 years old (n=47), 40-41 years old (n=37), and ≥ 42 years old (n=23) and age groups were compared. Results: The rate of live birth was %23.2 in 39 years and %14.7 in 40-41 years. No live birth in 42 years or older patients was recorded. The results revealed that that the age, number of retrieved oocytes and fertilized oocytes predict the success rate of pregnancy. Conclusion: Pregnancy rate dramatically decreased after the age of 39; the success of pregnancy after the intracytoplasmic sperm injection decreases 9 times, three years later starting age of 39. Purpose Endometrial thickness is important for implantation. Little data addresses the etiology of persistently thin endometrium. We present a patient cohort in order to define common features and draw conclusions. Methods Thirteen out of 1,405 IVF patients repeatedly had thin unresponsive endometrium (<7 mm). Age, history, uterine cavity status, treatment type and outcome were examined. Results Patient age was 35.9±5.7 years. Ten patients had a curettage performed previously. Nine patients had normal cavity and endometrium, and in four adhesions were diagnosed and removed. Out of 99 cycles performed afterwards, endometrial thickness increased in 22. ETs were performed in 49 cycles resulting in 11 pregnancies. Their outcome was eight miscarriages, two terminations due to malformations, and one live birth. Conclusions Thin unresponsive endometrium was associated with curettage, not necessarily with intrauterine adhesions. Even if adequate thickening eventually occurred, the reproductive outcome was still very poor. Therefore other alternatives should be sought for these patients.
