RESUMEN. Antecedentes: Se requiere analizar la situación de salud bucal en grupos vulnerables desde la perspectiva de los determinantes sociales para establecer estrategias de intervención efectivas. Objetivo: Describir el estado de salud bucal y factores relacionados en un grupo de mujeres en situación de prostitución en la ciudad de Medellín (Colombia). Métodos: Este fue un estudio descriptivo en una muestra por conveniencia de 53 mujeres que ejercen la prostitución. Se aplicó encuesta y examen clínico y se analizaron variables de autopercepción de salud bucal, índice COP-D (cariados, obturados y perdidos), índice de caries significativa (SiC), problemas de articulación temporomandibular (ATM) y estado protésico. Resultados: 88,2 % de las mujeres encuestadas reportaron un mal estado de salud bucal, 81 % se sentían insatisfechas con su estado bucal y 60 % reportó problemas bucales. La prevalencia de caries dental fue del 64 %, con un COP-D de 15,6 (±8,4), y un SiC de 25,5 (±3,6). Se presentaron diferencias en el estado de salud bucal en indicadores clínicos y subjetivos según factores sociodemográficos. Más de tres cuartas partes requerían cambia o realizar nuevas prótesis superiores o inferiores. En casi la mitad se hallaron ruidos articulares en la ATM según evaluación clínica. Conclusiones: El estado de salud bucal según los indicadores analizados es reflejo de las condiciones sociales en que se encuentran estas mujeres. Se encontraron diferencias en los indicadores de salud bucal de acuerdo con diferentes factores sociodemográficos, lo cual sugiere la influencia de los determinantes sociales en las desigualdades en salud bucal.ABSTRACT. Background: The analysis of the oral health situation in vulnerable groups from a perspective of social determinants is required to implement effective intervention strategies. Purpose: To describe the oral health status and its related factors in a group of women in situation of prostitution in Medellín (Colombia). Methods: A descriptive study was conducted with a convenience sample of 53 women. A survey and clinical examination was carried out. The study analyzed variables related to self-perceived oral health, DTMF Index, Significant Caries Index (SiC), temporomandibular joint problems (TMJ), and the status of fixed/removable prosthesis. Results: 88.2 % of the surveyed women reported their oral health status was poor, 81 % of them felt unsatisfied with their mouth, and 60 % reported oral problems. The prevalence of dental caries was 64 %, with a DTMF of 15.6 (± 8.4), and a SiC of 25.5 (± 3.6). There were differences in oral health status in clinical and subjective indicators according to sociodemographic factors. More than three-quarters required changing or making new upper and/or lower partial or fixed dentures. In almost half, articular noises were found in TMJ according to the clinical evaluation. Conclusions: The oral health status, from the basis of the indicators analyzed, reflects the social conditions found in these women. Differences in the oral health indicators were in accordance to sociodemographic factors. This situation suggests that there is an influence of social determinants on oral health inequalities.

The aim of this study was to measure the prevalence of gastrointestinal parasites (GI) in domestic animals and children in high mountain populations in the districts of Combia and Toche, Valle del Cauca–Colombia. These communities have been affected by the armed conflict in Colombia and are susceptible to health risk factors related to the Colombian post-conflict. Prevalence and risk factors were measured using Bayesian methods on 45 structured interviews applied to 29 families in Combia and 16 in Toche. This inquire aimed to analyze the socio-economic and demographic factors associated with the presence of parasites. This interview was conducted with 50 children: 40 (80%) from Rita Sabogal school district of Toche, and 10 (20%) from Tablones—Atanasio Girardot schools. 23 faecal samples from asymtomatic children from these schools were collected. Subsequently, 308 animals were characterized through the analysis of 64 faecal samples from asymptomatic individuals (20,8%); 18/41 from dogs (43,9%), 18/175 from poultry (10,3%), 7/13 from cats 56,84%, 6/20 from equines (30%) and from 15/59 cattle (25,43%). The prevalence of intestinal parasites among children under six years was 60% [95% PI = 41%-78%]; Endolimax nana , 24% [95% PI = 9,8%-42%]; Iodamoeba buetschlii , 16% [95% PI = 4,7%-32%]; Entamoeba coli , 35% [95% PI = 18%-55%]; Giardia lamblia , 12% [95% PI = 2,7%-27%]. In Equids the presence of Strongylus spp was 37% [95% PI = 10%-71%]; Parascaris equorum , 37% [95% PI = 10%-71%]; in dogs, Dipylidium caninum was 20% [95% PI = 6%-39%]; Trichuris trichiura , 9% [95% PI = 1,3%-26%]; Toxocara canis , 25% [95% PI = 9%-46%]; in cats, Toxocara cati had a prevalence of 44% [95% PI = 16%-75%]; cyst of Eimeria spp, 15% [95% PI = 3,4%-33%]; in poultry and Eimeria zuernii in cattle, 50% [95% PI = 23%-77%]. There was no association with exposure of humans to animal parasites. However, we conclude that female and children under 6 years of age are more likely OR (6,72–2,3) to get parasites.

Perception of effective access to health services in Territorial Spaces for Training and Reincorporation, one year after the peace accords in Colombia: a cross-sectional study

Backgrounds: The signing of the peace accords in Colombia created challenges that are inherent to post-conflict transitions. One of those is the process of reintegrating ex-combatants into society, in which ensuring their rights to health is a particularly significant challenge in rural areas affected by armed conflict. These areas, known as Territorial Spaces for Training and ReintegrationReincorporation (ETCR, in Spanish), are geographically dispersed throughout 24 municipalities and 13 departments in Colombia. This study aimed to describe how ex-combatants in ETCR regions perceived access to health services one year after the signing of the peace accords. Methods: A descriptive, cross-sectional study was performed between September and October 2018. It included 591 adults and their families, from 23 ETCRs. The study was designed, culturally validated, and piloted. Interviewers were trained and a structured survey was administered containing five dimensions that characterized the perception of effective access to health services. Results: The majority of interviewees were women, heads of household, young adults, ex-combatants, and residents in an ETCR. In total of 96.4% were enrolled in Colombia’s subsidized health system, and 20.8% indicated that a member of their household required emergency health services. The regional health center provided the majority of the services. Most of those surveyed (96.0%) reported that they did not have to pay for the services, and that they received respectful (91.6%) and good quality (66.6%) care. There were few referrals to disease prevention and health promotion activities, and only 19.0% of households reported having been visited by extramural health care teams, whose activities were highly valued (80%). Lastly, there was little knowledge about community health activities. Conclusions: While residents of ETCR regions have a favorable perception of their access to health services, they need to be made aware of extramural and public health activities.
