Skin Cancer Epidemics in the Elderly as An Emerging Issue in Geriatric Oncology

Skin cancer is a worldwide, emerging clinical need in the elderly white population, with a steady increase in incidence rates, morbidity and related medical costs. Skin cancer is a heterogeneous group of cancers comprising cutaneous melanoma and non-melanoma skin cancers (NMSC), which predominantly affect elderly patients, aged older than 65 years. Melanoma has distinct clinical presentations in the elderly patient and represents a challenging question in terms of clinical management. NMSC includes the basal cell carcinoma and cutaneous squamous cell carcinoma and presents a wide disease spectrum in the elderly population, ranging from low-risk to high-risk tumours, advanced and inoperable disease. Treatment decisions for NMSC are preferentially based on tumour characteristics, patient’s chronological age and physician’s preferences and operational settings. Several treatment options are available for NMSC, from surgery to non-invasive/medical therapies, but patient-based factors, such as geriatric comorbidities and patient’s life expectancy, do not frequently modulate treatment goals. In melanoma, age-related variations in clinical management are significant and may frequently lead to under-treatment, limiting access to advanced surgical and medical treatments. Clinical decision-making in the care of elderly skin cancer patient should ideally implement a geriatric assessment, prioritizing patient-based factors and efficiently differentiating fit from frail cancer patients. Current clinical practice guidelines for NMSC and melanoma only partially address geriatric aspects of cancer care, such as frailty, limited life-expectancy, geriatric comorbidities and treatment compliance. We review the recent evidence on the scope and problem of skin cancer in the elderly population as well as age-related variations in its clinical management, highlighting the potential role of a geriatric approach in optimizing dermato-oncological care.

“Nevi of special sites” is a term that denotes melanocytic nevi presenting in specific anatomic locations including the scalp, genital area, flexural sites, and acral sites [1]. Nevi from these anatomic sites display at times histopathologic features that may lead the reading pathologist to recommend re-excision of these benign nevi. Reflectance confocal microscopy (RCM) is a noninvasive imaging tool that allows for visualization of epidermal, dermal-epidermal junctional (DEJ), and superficial dermal tissue structures at cellular level resolution. RCM features of special site nevi have not been previously described in the literature. Defining the RCM characteristics of special site nevi may increase diagnostic accuracy and assist in ruling out melanoma. Here, we report a case of a pigmented lesion appearing in the axilla of a patient with a recently diagnosed melanoma. Dermoscopic and histopathologic results were consistent with the diagnosis of nevus in flexural anatomic sites. In this case, RCM showed a regular honeycomb pattern of epidermal keratinocytes and enlarged, non-homogenous, discohesive nests at the DEJ, a pattern that corresponded well with the histopathologic findings. Larger studies are needed to establish RCM features of special site nevi in order to reliably rule out melanoma and lower the rate of unnecessary excisions of these benign nevi.

Background Anthropogenic climate change affects the burden of infectious diseases via several interconnected mechanisms. In recent years, there has been greater awareness of the ways in which climate-sensitive infectious diseases pose a growing threat to global public health. Objective This study aimed to categorize and describe the effects of climate change on infectious diseases with skin manifestations. Methods A scoping review of the MEDLINE and PubMed online databases for climate-sensitive infections was performed in February and March 2020. A representative selection of conditions with skin manifestations was included in this review. Results Several representative climate-sensitive infectious diseases were identified in each of the following categories: vector-borne infectious diseases, infectious diseases associated with extreme weather events, and infectious diseases linked to human migration. Conclusion Climate variables directly influence the survival and reproduction of infectious microorganisms, their vectors, and their animal reservoirs. Due to sustained warmer temperatures at higher latitudes, climate change has expanded the geographic range of certain pathogenic microbes. More frequent climate change-related extreme weather events create circumstances where existing infectious microorganisms flourish and novel infections emerge. Climate instability is linked to increased human migration, which disrupts health care infrastructure as well as the habitats of microbes, vectors, and animal reservoirs and leads to widespread poverty and overcrowding. Dermatologists should understand that climate change will affect the burden and geographic distribution of infectious diseases, many of which have cutaneous signs and might be encountered in their regular practice.
