Emerging echinocandin-resistant Candida albicans and glabrata in Switzerland

Abstract: Echinocandins represent the first-line therapy of candidemia. Echinocandin resistance among Candida spp. is mainly due to acquired FKS mutations. In this study, we report the emergence of FKS-mutant Candida albicans/glabrata in Switzerland and provide the microbiological and clinical characteristics of 9 candidemic episodes. All patients were previously exposed to echinocandins (median 26 days; range 15–77). Five patients received initial echinocandin therapy with persistent candidemia in 4 of them. Overall mo… Show more

“…Our findings may alter the awareness of non- albicans Candida species and their spectrum of therapeutic resistance in the general care of elderly people. Due to C. glabrata increasingly gaining importance in several patient groups, such as abdominal surgery patients, or due to the occurrence of echinocandin resistance in this species [ 36 , 37 , 38 ], our findings should be taken into account when treating elderly patients. Screening for oral [ 39 ] or gastrointestinal fungal colonization prior to surgical intervention or when employing indwelling devices might be a tool to facilitate adequate subsequent calculated antifungal therapy [ 1 ].…”

“…Overall, the rates of echinocandin treatment failure for candidemia due to FKS -mutant Candida spp. ranges from 60 to 90% [ 51 , 52 , 54 , 55 ], while the usual failure rate of echinocandins in the big trials of candidemia is around 10 to 30% [ 56 , 57 , 58 , 59 ].…”
