Elective radical neck dissection in epidermoid cancer of the head and neck. A retrospective analysis of 853 cases of mouth, pharynx, and larynx cancer

Abstract: A retrospective analysis of 853 patients with cancer of the mouth, pharynx, and larynx operated on over a 30-year period was performed. Four hundred fifty-seven of them had a radical neck dissection (RND) at some point. Five hundred ninety patients had no clinically positive nodes (N-o) necks at the time of primary treatment; 99 of these had elective neck dissection, whereas 95 others had a delayed RND when nodes became clinically involved. Twenty-three percent of all N-o patients had microscopically involved … Show more

“…The incidence of occult disease in the lymph nodes after initial treatment ranges from 13% to 33% and 27%–53% in Stage I and II, respectively. [231011121314] The rate of occult metastases in regional neck nodes in our study is 33%. Therefore, strengthening the fact that clinical palpation is grossly inadequate for evaluation of N0 neck in oral SCC.…”

“…Management of the neck with observation requires careful follow up with highly reliable imaging [75]. Some studies have reported a higher survival rate with observation than with elective neck dissection [74,76,77].…”

“…In the literature, the reported salvage rate varies from as low as 27% to 82% (Average, 50%) after a regional recurrence (Table 6). [3][4][5][6][7][8][9][10][11] In this study we found a salvage rate of 100%, although our followup period is still limited.…”

“…[3][4][5][6][7][8][9][10][11] (Table 6). In all these studies, in which only palpation is used to initially stage the neck, the recurrence rate in the neck is between 24% and 57% (Mean 32%) ( Table 6).…”
