RESUMOO câncer de esôfago é o oitavo tipo de neoplasia maligna mais incidente em âmbito mundial, constituindo a terceira causa de morte por câncer no sistema digestivo. Dentre os fatores de risco, o tipo adenocarcinoma está atrelado à doença do refluxo gastroesofágico e ao esôfago de Barrett. O carcinoma epidermóide, por sua vez, está associado ao menor nível socioeconômico e à história pregressa de tabagismo e etilismo. O objetivo do presente trabalho consiste em caracterizar as internações hospitalares por câncer de esôfago no município de Governador Valadares (Minas Gerais), entre os anos de 2011 e 2020. Trata-se de um estudo ecológico com dados secundários de domínio público disponíveis no Portal de Vigilância da Secretaria de Estado de Saúde de Minas Gerais. Para caracterização, as informações consideradas foram: ano de internação, sexo, faixa etária, óbitos, caráter de atendimento, dias de permanência e gastos. Totalizaram-se 1.892 internações hospitalares por câncer de esôfago em todo o período estudado, com destaque para o caráter de urgência (n=1.081; 95,1%). Quanto ao regime, a maioria das fichas continha essa informação ignorada (n=1.037; 52,3%); enquanto 38,4% (n=761) das internações ocorreram sabidamente no sistema privado e 94 (4,7%) no sistema público. Frisam-se ainda sexo masculino (n=1.452; 76,7%) e faixas etárias 60 a 69 anos (n=595; 31,4%) e 50 a 59 anos (n=608; 26,8%). Ademais, 288 casos evoluíram para óbito (letalidade hospitalar de 15,2%). O perfil de falecimentos foi semelhante ao de internações, com cerne para homens (n=223; 77,4%) e idade 60 a 69 anos (n=82; 28,5%). A letalidade hospitalar foi levemente mais expressiva no sexo masculino (15,4%) comparando-se ao feminino (14,8%), porém em se tratando da idade, foi proeminente entre indivíduos de 80 anos ou mais (19,1%). Do ponto de vista temporal, o menor número de hospitalizações por câncer de esôfago foi registrado em 2011 (n=155; 8,2%), atingindo o maior valor em 2016 (n=266; 14,0%) e concluindo a série histórica em 2020 com 180 (9,5%) internações. Trimodality based on neoadjuvant chemoradiotherapy (nCRT) followed by surgery is gaining popularity as a treatment strategy for locally advanced esophageal cancer. In this review, we summarize the role of nCRT and the recommended nCRT regimens based on clinical trials and meta-analyses. We analyze the relationship of nCRT with pathologic complete response (pCR) and then identify potential predictive markers of response. Compared with surgery alone and neoadjuvant chemotherapy followed by surgery, trimodality provides longer survival and has the advantage of local control compared with definitive chemoradiotherapy. The standard regimen is a platinum-based regimen with a radiation dose range of 41.4-50.4 Gy by conventional fractionation. Evidence shows that patients with pCR tend to live longer than non-responders, indicating that pCR is a significant prognostic factor for patients with esophageal cancer. Individualized medicine requires predictive markers of individual patients based on their own genes. Currently, no definite marker is proved to be sufficiently sensitive and specific for use in clinical practice, although 18-fluorodeoxyglucose positron emission tomography shows promise in predicting response to nCRT.

Background:Esophageal squamous cell carcinoma is an aggressive neoplasia that requires a multidisciplinary treatment in which survival and prognosis are still not satisfactory. The complete pathologic response to neoadjuvant chemotherapy and radiotherapy is considered a good prognosis factor, and esophagectomy is indicated. Aim:Survival analysis of cases with pathologic complete response (ypT0 ypN0) to neoadjuvant chemotherapy and/or radiotherapy, submmitted to esophagectomy. Methods:Between 1983-2014, 222 esophagectomies were performed, and 177 were conducted to neoadjuvant treatment. In 34 patients the pathologic response was considered complete. Medical records of the patients were retrospectively reviewed regarding type of chemotherapy applied, amount of radiotherapy, interval between the neoadjuvant therapy and the surgery, body mass index; postoperative complications; hospital admission time and survival. Results:The average age was 55.8 years. Twenty-five patients were subjected to chemotherapy and radiotherapy, and nine to neoadjuvant radiotherapy. The total radiation dose ranged from 4400 until 5400 cGy. The chemotherapy was performed with 5FU, cisplatin, and carbotaxol, concomitantly with the radiotherapy. The esophagectomy was transmediastinal, followed by the cervical esophagogastroplasty performed on a average of 49.4 days after the neoadjuvant therapy. The hospital admission time was an average of 14.8 days. During the follow-up period, 52% of the patients submitted to radiotherapy and chemotherapy were disease-free, with 23.6% of them presenting more than five years survival. Conclusions:The neoadjuvant treatment followed by esophagectomy in patients with pathologic complete response is beneficial for the survival of patients with esophageal squamous cell carcinoma.
