SummaryBackground/purpose: Among the primary cutaneous T-cell lymphomas, mycosis fungoides (MF) is the most common disease entity. Recently, an improved understanding of the pathology, clinical presentation, and prognosis of MF has lead to the development of new and practically useful classification and staging systems. In most patients, MF presents with patches and plaques and remains confined to the skin for years and decades, making it an ideal target for phototherapy. However, treatment schedules vary widely and this review describes the current knowledge about phototherapy of MF focusing mainly on narrow-and broadband UVB and 8-methoxypsoralen plus UVA, its indications, practical aspects, and clinical outcome. Methods: Review and summary of the pertinent literature. Results and conclusions: Since 1976, when the first report on phototherapy for MF was published, sufficient evidence has accumulated to make narrowband UVB and PUVA safe and effective treatment options for early stages of the disease. In refractory cases or more advanced stages, combination of phototherapy with systemic treatments including mainly interferons and retinoids might be valuable. Additional research is required to further define the optimal treatment schedules and the role of maintenance.

“…Besides the standard skin-directed therapies such as phototherapy with PUVA or narrowband ultraviolet B, topical steroids, topical chemotherapy with nitrogen mustard or carmustine, bexarotene gel, and total skin electronbeam therapy, new treatment approaches with alternative retinoids such as tazarotene and topical immunomodulators such as imiquimod have shown efficacy in a small number of patients with early CTCL [29,30]. Moreover, treatment with 308-nm excimer laser might be an option in selected instances resistant to phototherapy [31,32]. A reported surgical excision of palpebral tumors in a patient with d'emblee type of CTCL provided significant cosmetic and functional improvement and may represent an effective adjunct therapy in selected cases with localized tumors [33].…”

Zusammenfassung Kutane T-Zell-Lymphome (CTCL) stellen eine heterogene Gruppe extranodaler Non-Hodgkin-Lymphome dar. ?berwiegend handelt es sich bei ihren Vertretern um chronische, niedrig-maligne Lymphome, die mit einer g?nstigen Prognose verbunden sind. Andererseits ist auch durch hochaggressive medikament?se Ma?nahmen eine kurative Therapie in der Regel nicht erzielbar. Erkenntnisse zu den Ursachen der einzelnen CTCL-Formen sind nach wie vor spekulativ. Die verbesserte Klassifikation der CTCL wie auch die Entwicklung neuer Medikamente in den letzten Jahren hat zu differenzierten krankheits- und stadiengerechten Therapieoptionen gef?hrt, die f?r viele Patienten eine gute Beherrschung ihres Krankheitsbildes m?glich macht. Der Artikel beschreibt den aktuellen Stand der Diagnostik und Therapie der CTCL und gibt einen Ausblick auf innovative in der Entwicklung befindliche Behandlungsans?tze.
