Objective: We investigated changes in cortico–subcortical spatiotemporal dynamics to explore the treatment mechanisms of transcranial alternating current stimulation (tACS) in patients with Parkinson’s disease (PD). Methods: Resting-state functional magnetic resonance imaging (rs-fMRI) data were collected from 20 patients with PD and 20 normal controls (NC). Each patient with PD received successive multidisciplinary intensive rehabilitation treatment and tACS treatment over a one-year interval. Individual functional brain network mapping and co-activation pattern (CAP) analysis were performed to characterize cortico–subcortical dynamics. Results: The same tACS electrode placement stimulated different proportions of functional brain networks across the participants. CAP analysis revealed that the visual network, attentional network, and default mode network co-activated with the thalamus, accumbens, and amygdala, respectively. The pattern characterized by the de-activation of the visual network and the activation of the thalamus showed a significantly low amplitude in the patients with PD than in NCs, and this amplitude increased after tACS treatment. Furthermore, the co-occurrence of cortico–subcortical CAPs was significantly higher in patients with PD than in NCs and decreased after tACS treatment. Conclusions: This study investigated cortico–subcortical spatiotemporal dynamics in patients with PD and further revealed the tACS treatment mechanism. These findings contribute to understanding cortico– subcortical dynamics and exploring noninvasive neuromodulation targets of cortico–subcortical circuits in brain diseases, such as PD, Alzheimer’s disease, and depression.

Quality of life predicts rehabilitation prognosis in Parkinson's disease patients

Background: Rehabilitation has been reported to improve the quality of life (QoL) of patients with Parkinson's disease (PD). Nevertheless, not all patients are satisfied with rehabilitation outcomes and could achieve a significant improvement in QoL.Objective: To detect possible predictors of QoL improvement in patients with PD after rehabilitation.Methods: A total of 86 PD patients were included and followed up for 3 months with a 39-item Parkinson's Disease Questionnaire summary index (PDQ-39 SI) as the primary endpoint. All patients received 2 weeks of multidisciplinary intensive rehabilitation treatment (MIRT). Changes in patients' QoL were assessed using the PDQ-39 at baseline and at the 3-month follow-up. The reliable change index (RCI) was adapted to determine the individual QoL outcome. The predictors of QoL outcome were detected using logistic regression analysis.Results: After a 3-month follow-up, PDQ-39 SI decreased significantly from 22.95 ± 9.75 to 18.73 ± 10.32 (P < 0.001). Scores for QoL improved (RCI>10.9) after rehabilitation for 18.6% of the patients, and 74.4% of patients reported an unchanged QoL (−10.9≤RCI≤10.9), while 7.0% of patients reported a worsening of QoL (RCI<−10.9).

Long-term use of dopaminergic therapy in Parkinson's disease (PD) is associated with reduction in efficacy and disabling dyskinesias. The current medical or surgical treatment modalities are ineffective for atypical parkinsonism syndromes. Hence, there is a need for holistic and cost-effective non-pharmacological interventions that act via multiple mechanisms to improve motor as well as non-motor symptoms among PD patients. Rehabilitation strategies focusing on multiple mechanisms can lead to improvement in certain symptoms among PD patients, which may be refractory to medical and surgical therapy. However, there is scanty literature available on the role of various rehabilitation strategies in patients with atypical parkinsonism patients. Multiple rehabilitation strategies such physiotherapy, aerobic exercises, strength/resistance exercises, treadmill training, cueing, dance and music, speech language therapy, occupational therapy, hydrotherapy, and martial arts have been found to improve motor as well as non-motor symptoms among PD patients. Newer modalities such as virtual-reality-based devices, exergaming, wearable sensors, and robotic prosthetic devices may be exciting future prospects in rehabilitation among patients with PD and atypical parkinsonian syndromes. This narrative review assessed and summarized the current evidence regarding the role of various rehabilitation strategies in PD and atypical parkinsonian syndromes. Furthermore, evidence regarding recent advancements in rehabilitation for patients with parkinsonism was highlighted. Despite the beneficial effect of rehabilitation in PD, there is still scanty literature available from India on rehabilitation strategies among PD patients. Larger prospective randomized control trials from India and other low- and middle-income countries, focusing on various rehabilitation strategies among PD patients, are an unmet need.
