Alopecia has a significant negative impact on the quality of life. Unfortunately, there is no satisfactory cure for most types of alopecia. Alopecia is divided into cicatricial and noncicatricial types. Androgenetic alopecia, alopecia areata, and telogen effluvium are common forms of noncicatricial alopecias. In order to treat or improve the appearance, various procedures that are being applied for different types of alopecia including mesotherapy, microneedling, platelet-rich plasma, low-level light therapy, and stem-cell therapy with variable outcomes are reviewed in this chapter.

Background: Platelet-rich plasma (PRP) is an autologous preparation of platelets in concentrated plasma that may be useful in the treatment of androgenetic alopecia (AGA). Its action depends on the released growth factors from platelets. Skin needling is a technique that uses a sterile dermaroller to puncture the skin and release growth factors. The combination of Skin needling and PRP should enhance the efficacy of both modalities. Objective: To establish the effectiveness of the combined use of Skin needling and PRP in AGA treatment. Method: The study enrolled 10 patients of AGA. The hair pull test was performed before each treatment session. PRP therapy with skin needling was given to them at monthly intervals for 6 months. The effects of therapy were evaluated based on clinical examination, hair pull test, photographic assessment and patient's satisfaction. Result: Before treatment, 80 % of our patients had a positive hair pull test with a mean number of 9 hairs which turned to be negative only after the third session. After 6 months, there was an overall improvement in hair density with the response of grade +2 in 8 patients and +1 in 2 patients on the standardised 7-point evaluation scale. 3 patients rated response as grade 4, 5 patients as grade 3 and 2 patients as grade 2 on patients' subjective evaluation of hair growth. Pain during the procedure and sensible scalp afterwards were the major complaints which subsided in 2-3 days. Conclusion: PRP combined with skin needling is a safe and effective treatment and can be considered as an alternative for management of AGA.

ÖZKondromalazi patella, patella arkasındaki kıkırdağın yumuşaması ve ilerleyen süreçte ülserleşmesiyle devam edebilen, diz önü ağrısı ile karekterize bir hastalıktır. Tedavisinde klasik fizik tedavi ve rehabilitasyon yaklaşımları, ağrı kesici ilaçlar, hasta eğitimi gibi tedavi yaklaşımları yer almaktadır. Son yıllarda kıkırdak lezyonlarında doku rejenerasyonu amacıyla PRP (Platelet-Rich Plasma) tedavisi daha da artan bir sıklıkta kullanılmaya başlanmıştır. Bu olgu sunumunda PRP tedavisi ile başarılı sonuç alınan kondromalazi patella tanılı bir hastayı sunmayı amaçladık.Anahtar kelimeler: Kondromalazi patellada, Platelet-Rich Plasma, diz ağrısı ABSTRACT Chondromalacia patellae is characterized by anterior knee pain and softening of the cartilage behind the patella, which may further lead to cartilage ulceration. Conventional physical therapy and rehabilitation treatment, pain medications, and patient education are the current management options. Platelet-rich plasma (PRP) preparations have been more commonly used in the management of cartilage lesions for regenerating tissues in the recent years. We aimed to present a patient with chondromalacia patellae who was successfully treated with PRP at our clinic.Keywords: Chondromalacia patellae, Platelet-Rich Plasma, knee pain Olgu Sunumu / Case Report GİRİŞ Kondromalazi patella, patellar kartilajın yumuşaması ve minimal dejenerasyonundan geniş ülsere alanların oluşmasına kadar ilerleye bilen bir histopatolojik tabloda, diz önü ağrısıyla karekterize, toplumda sık görülen bir hastalıktır (1, 2). Ağrı, şişlik, retropatellar krepitasyon gibi bulgular ile kendini gösteren bu rahatsızlık genç populasyonda ve kadın cinsiyette nispeten daha fazla görülür (3).Kondromalazi patella tedavisinde sistemik ve topikal farmakoljik ajanların yanında yüzeysel ve derin ısıtıcılar, aneljezik elektrik akımları, manüplatif tedavi, egzersiz gibi fiziksel tıp ve rehabilitasyon yöntemlerinden yararlanılmaktadır (4-6).Son yıllarda gittikçe daha fazla uygulanan, diş hekimliğinden, dermokozmetolojiye ve kas iskelet sistemi hastalıklarına kadar geniş bir yalpezede uygulama alanı bulan PRP tedavisi her geçen gün üzerinde daha fazla çalışılan bir yöntem olarak dikkat çek-mektedir (7,8). Bizde kliniğimizde PRP tedavisi ile başarılı sonuç alınan kondromalazi patella tanılı olguyu vaka takdimi şeklinde tartışmayı amaçladık. Hastaya kondromalazi patella tanısı ile 1'er ay arayla 3 doz PRP tedavisi uygulandı. Hazırlanan PRP sıvısı Im ve arkadaşlarının literatüründe tarif edildiği şekilde patellofemoral eklem aralığına enjekte edildi (Resim 1) (9). Ayrıca ev egzersiz programı verildi. Tedavi süresince ağrı kesici olarak sadece parasetamol kullanması önerildi. Hasta 3. enjeksiyondan 1 ay sonra tekrar değerlendirildi. Tedavi sonunda hastanın ağrı yakınmalarında belirgin gerileme (VAS=2) ve fonksiyonel durumunda belirgin düzelme (total WO-MAC skoru= 29) gözlendi. OLGU SUNUMU
